HICAP (the Health Insurance Counseling & Advocacy Program)

HICAP (the Health Insurance Counseling & Advocacy Program) provides free and objective information and counseling about Medicare. Volunteer counselors can help you understand your specific rights and health care options. HICAP also offers free educational presentations to groups of Medicare beneficiaries, their families and/or providers on a variety of Medicare and other health insurance related topics.

POLST Form

POLST: Portable Medical Orders, or POLST for short. Portable means that the order is valid outside the clinic or doctor’s office, similar to a drug prescription. POLST communicates your wishes as medical orders.

Rohnert Park Senior Center/ Meals on Wheels

Centrally located in Sonoma County, the City of Rohnert Park offers residents and visitors many wonderful amenities, including the Senior Center. The Senior Center offers special events, trips, and plenty of activities with an ongoing schedule.

The Meals on Wheels Program successfully addresses the risks of hunger and isolation among temporarily or permanently homebound seniors who are not able to provide for themselves without assistance. The program delivers a fresh meal containing one third of the recommended daily requirement 5 days a week.

Council on Aging / Meals on Wheels

Council on Aging Services for Seniors is a private non-profit organization which has been providing services for Sonoma County citizens over the age of 60 since 1966. The area’s beautiful surroundings, safe atmosphere, and senior housing have attracted over 137,000 older adults to this community. In fact, Santa Rosa is ranked 6th in the nation for cities having the highest population over 85 years of age.

Since 1977, Council on Aging has operated the largest meal program for seniors in Sonoma County. The Council on Aging Senior Nutrition Services provides over 285,000 meals annually to seniors in need in the following five programs: Meals on Wheels, Congregate Dining, Senior Social Club/Adult Day Respite Program, and Weekend Meals. These meals reach our most vulnerable, food insecure seniors. The meals provide one third of the recommended dietary requirements for seniors. For many clients, it is their only meal of the day.

Redwood Caregiver Resource Center - Grants for Care

In operation since 1985, Redwood Caregiver Resource Center continues to provide services to family caregivers throughout the Redwood Empire, including the counties of Del Norte, Humboldt, Mendocino, Lake, Sonoma, Napa, and Solano. Redwood CRC is part of a statewide system of eleven nonprofit Caregiver Resource Centers (CRCs) offering support and assistance to caregivers of adults with brain impairments through education, research, services and advocacy. Funding comes from the California Department of Mental Health, grants, fees for services, and gifts from our generous donors. All services are free or low cost.

Redwood Empire Food Bank - Santa Rosa

Since 1987, the Redwood Empire Food Bank has been feeding our neighbors in need and leading the community to end hunger. We distribute food through our own programs and our partner organizations in Sonoma, Lake, Mendocino, Humboldt, and Del Norte counties. As the largest hunger-relief organization in our area, we work on the front lines of emergency food assistance in our region, playing a crucial role in helping individuals, families, seniors, and children.
