Nurse-directed care, built around the life you want.
Every person’s aging journey is different. That’s why nurse-directed care begins the same way all Home Instead care does—by getting to know you. We take time to understand your routines, concerns, recent changes, and what matters most in your daily life.
A registered nurse connects with you early in the process to assess your needs, answer questions, and help determine if nurse-directed care is the right fit. From there, the Home Instead San Antonio, TX develops a personalized care plan, and a registered nurse provides guidance to specially trained Care Pros who carry it out with consistency and respect.
It’s care that goes beyond the basics—while staying focused on what matters most: helping you live safely and independently at home.

Nurse-directed Care FAQs
Nurse-directed care is a level of in-home health care that's overseen and guided by a registered nurse. A licensed RN creates and manages your care plan and trains our Care Pros to assist with more medically focused tasks, like medication management, wound care, and glucose monitoring. This is different from skilled nursing, where an RN delivers all the medical care directly. With nurse-directed care, your RN is the guide, and your Care Pro is there to carry out the plan with consistency and compassion.
Home Instead Care Pros are carefully screened, background-checked, and supported with education based on our person-centered care approach—including support for dementia, mobility, and personal care. We also prioritize matching you with someone compatible.
