Care that puts your recovery first.
Here’s how transitional care can help:
- Quick placement – care often begins within 24 hours, helping make the transition home or to a rehab facility smoother.
- Encouragement to stay engaged – gentle motivation to participate in therapy, movement, and daily routines.
- Medication and appointment help – from picking up prescriptions to helping you get to follow-up appointments, we help take care of the everyday tasks.
- Support for healthy routines – help with meals, hydration, rest, and daily structure.
- Reduced isolation – meaningful conversation can greatly help improve mood during a vulnerable time.
- Added safety and supervision – extra support to help reduce risks that may lead to setbacks or unnecessary hospital readmissions.

Transitional Care FAQs
Acting quickly is usually a necessity when someone is discharged. Our Care Pros can accompany clients to the ER, stay with them during a hospital stay and be in place on discharge day. This often begins as around-the-clock care for the first 24 to 48 hours and transitions to a preferred schedule as the client regains confidence.
When a client was discharged under a 'hospital from home' program for pneumonia — a arrangement requiring support at home — and had no family able to meet that need, we arranged around-the-clock care within 24 hours. He received hospital-level treatment safely from his own home.
Many clients and their families feel anxiety on discharge day — about whether the home is safe, whether care will hold up overnight and whether they are ready. Having a Care Pro present from the moment of arrival, knowing who it is and what the plan covers, addresses that anxiety directly.
Our Care Pros are also experienced in accompanying clients to the ER and staying with them during a hospital stay, so the transition home continues a relationship rather than starting a new one.
