Care that puts your recovery first.
Here’s how transitional care can help:
- Quick placement – care often begins within 24 hours, helping make the transition home or to a rehab facility smoother.
- Encouragement to stay engaged – gentle motivation to participate in therapy, movement, and daily routines.
- Medication and appointment help – from picking up prescriptions to helping you get to follow-up appointments, we help take care of the everyday tasks.
- Support for healthy routines – help with meals, hydration, rest, and daily structure.
- Reduced isolation – meaningful conversation can greatly help improve mood during a vulnerable time.
- Added safety and supervision – extra support to help reduce risks that may lead to setbacks or unnecessary hospital readmissions.

Transitional Care FAQs
Our Return to Home program is designed for families who need help when an older adult is discharged from a hospital or rehabilitation setting. The support can be tailored to what the person needs when they return home.
That may include help with meals, personal care, mobility, transportation, medication reminders, household routines, and getting to follow-up appointments. The program reflects a need our Bloomington team sees frequently: families suddenly realizing that being medically ready for discharge doesn’t necessarily mean someone is ready to manage everyday life independently at home.
Home Instead Bloomington can typically start care within two to three days, depending on the person’s needs, schedule, and Care Pro availability.
If you know a discharge is approaching, contacting the team before the person comes home gives us more time to develop the Return to Home plan and arrange an appropriate Care Pro. That can be especially helpful when an older adult will need transportation, personal care, meals, mobility assistance, or help attending follow-up appointments after returning home.
Yes. One local veteran faced exactly that situation. After a fall, hospitalization, and rehabilitation stay, he needed increased support but didn’t have family living nearby.
Our Client Care Manager attended his discharge care plan meeting, and the team worked with home health and hospice as additional VA-authorized hours were pursued. Home Instead also arranged for someone familiar to be waiting at his home after discharge. For families coordinating care from a distance—or older adults without local relatives—that kind of practical support can make the transition less isolating.
Yes. Home Instead Bloomington can provide home care alongside home health when both are involved.
The roles are different. Home health handles appropriate clinical services, while a Home Instead Care Pro can assist with everyday needs included in the customized care plan. Our local team has firsthand experience coordinating around clients who are transitioning home and receiving services from other providers, including the veteran whose discharge required communication with home health and hospice.
Monroe County includes rural areas where getting to medical appointments, grocery stores, and other necessities can be challenging for an older adult who no longer drives or is temporarily limited after an illness or injury.
Home Instead Bloomington sees transportation as an important part of supporting local clients. Depending on the care plan, a Care Pro can help an older adult get to follow-up appointments and complete necessary errands while also assisting with the routines at home that may be more difficult during recovery.
