Your Role as a Client Care Coodinator:
  • Client Onboarding. Conduct in-depth assessments with prospective clients, develop and present care solutions and create initial plan of care by meeting directly with Seniors and their families at their homes to understand their specific care needs.
  • Quality Assurance. Visit clients' homes regularly to assess care needs firsthand, ensuring delivery of care consistently meets clients needs and expectations.
  • Client Communication. Maintain empathetic and effective communication with seniors and their families, ensuring their needs and concerns are addressed with care and understanding.
  • Caregiver & Client Resource. Respond to internal employees, clients and community members questions and requests for assistance, creating connections and offering solution based recommendations
  • Documentation. Ensure all interactions are clearly and concisely documented in a timely manner; follow up on commitments and complete tasks promptly to maintain high standards of service
  • Collaborate effectively within a team setting while also thriving independently to achieve optimal outcomes for our clients.
  • Utilize various software tools to enhance operational efficiency, facilitate seamless team communication and uphold our commitment to quality care delivery.
What We are Looking For:
  • BS Degree or relevant coursework in Gerontology, Social Work or Health Care Administration (or equivalent experience); MS Degree in related field a plus
  • Minimum of 2-3 years prior work experience in a service oriented role that includes face-to-face customer interaction and problem resolution
  • Demonstrated passion for helping others
  • Previous experience in senior care or related field (Hospice, Home Health, Hospital, Skilled Nursing, Rehabilitation Facility) is desirable.
  • Excellent verbal and written communication skills, with a demonstrated proficiency in using a consultative approach communication style; Bilingual Spanish a plus
  • Strong documentation skills with a focus on thorough follow-up and follow-through on all interactions
  • Ability to excel in a fast-paced environment, both as an individual and as part of a dynamic team
  • Proficiency in software programs for operational efficiency, communication and documentation
  • Demonstrated effective time management skills
