On-Call Coordinator

We are looking for someone who enjoys providing awesome customer service, a friendly warm welcoming voice over the phone and loves working with Seniors/CAREGivers in a high paced environment!

Our On-Call Coordinator is expected to perform a variety of on-call duties after business hours, including scheduling shifts and working closely with the Service Coordinators in order to provide the highest quality service to clients. This position would alternate every other week, Monday through Thursday from 5pm to 8am and Friday 5pm through the following Monday 8am. During this time you will be answering all incoming calls, fill all client shifts following designated office scheduling processes that come open, communicate all changes with client(s) and CAREGiver(s). Create a report at the end of shift with all call logs as well as logging all communication in client(s)/CAREGiver(s) profile, contact backup support if needed, field and schedule Client/CAREGiver inquires and perform all other functions deemed necessary.

Must have an understanding of and uphold the policies and procedures established by Home Instead. Must be patient and congenial on the telephone. Must demonstrate effective interpersonal skills as well as sound judgment and independent decision-making skills. Must demonstrate knowledge of the senior care industry, met basic computer skills, verbal and written communication skills and the ability to listen effectively.

Client Care Coordinator

Job Details

Home Instead is looking for a caring and compassionate Client Care Coordinator to become part of our team and join our mission of enhancing the lives of aging adults throughout our community. Home Instead provides a variety of services that allow seniors to remain in their home and meet the challenges of aging with dignity, care, and compassion. 

Position Overview 

The Client Care Coordinator has overall responsibility for managing the client/family relationship and sales/service opportunities. The Client Care Coordinator is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiver introductions and quality assurance visits with existing clients. They will work with individuals and families to determine needs and provide solutions through a tailored personalized service plan. The Client Care Coordinator will continuously evaluate the plan through ongoing quality assurance visits to ensure the appropriate level of care and monitor potential condition changes. 

Essential Job Functions 

Overall responsibility for the client/family relationship with the office
Meet with potential clients and family members to discuss their needs and build a personalized service plan 
Conduct quality assurance visits and evaluate condition changes with existing clients
Participate in secondary on-call rotation 
Maintain effective communications with clients, CAREGivers, and office staff
Conduct CAREGiver introductions with new clients as needed
Maintain all client records and documentation of service agreements and updating when appropriate 
Work to resolve issues that may arise in a timely manner with clients and/or family members 

Qualifications 

College degree preferred 
One-year experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered
Must possess a valid driver's license

Director of Nursing

Job Details

Home Instead Senior Care is looking for a caring and compassionate Director of Nursing to become a part of our team and join our mission of enhancing the lives of aging adults throughout our community. Home Instead Senior Care provides a variety of services that allow seniors to remain in their home and meet the challenges of aging with dignity, care and compassion. 

Position Overview 

The Director of Nursing position will provide professional services and care to seniors through consultation, assessment, planning, and evaluation. This position will manage and lead our team of CAREGivers through training and nurse delegation to assure the highest standard of quality care and safety is given for our clients. 

Essential Job Functions

  • Establish working knowledge of all business policies and procedures, including those governing delegation, supervision, and training
  • Assessment of client's current health and medical history
  • Create, assess, evaluate, and update client plan of care as often as the severity of the client's condition requires 
  • Determine where delegation of nursing services is appropriate in accordance with applicable regulations, individual client condition, and each assigned team member's training and demonstrated competency, and maintain documentation of all factors evaluated in making the determination(s). 
  • Communicate with internal and external healthcare professionals 
  • Preform remote and on-site supervision of services by CAREGivers as needed to ensure compliance with quality of care, client safety, and state standards
  • Perform clinical services within Home Instead scope of care, that require a professional license in conjunction with our policies and procedures
  • Lead training efforts for CAREGivers regarding nurse delegated services  
  • Give thorough, concise reports to CAREGivers by communicating pertinent information in a timely manner
Qualifications 

A graduate from an accredited school of nursing, with a nursing degree is required. Must have at least three years’ experience in a clinical or home care setting. Prior home health and nurse delegation experience is desired. Previous position working with seniors is preferred. Must maintain a current license in good standing as a registered nurse in the state of Nebraska. Must possess a current CPR and BLS certification and managed care/case manager certification is a plus.
