Care that puts your recovery first.
Here’s how transitional care can help:
- Quick placement – care often begins within 24 hours, helping make the transition home or to a rehab facility smoother.
- Encouragement to stay engaged – gentle motivation to participate in therapy, movement, and daily routines.
- Medication and appointment help – from picking up prescriptions to helping you get to follow-up appointments, we help take care of the everyday tasks.
- Support for healthy routines – help with meals, hydration, rest, and daily structure.
- Reduced isolation – meaningful conversation can greatly help improve mood during a vulnerable time.
- Added safety and supervision – extra support to help reduce risks that may lead to setbacks or unnecessary hospital readmissions.

Transitional Care FAQs
We work directly with hospital discharge planners and aim to have a Care Pro ready on the day of discharge. That caregiver can assist with transportation home, mobility support, meals, medication reminders, follow-up appointment rides and reinforcing instructions from therapy or the medical team.
For clients who need durable medical equipment before returning home, we can help families connect with local resources to have what they need in place on arrival.
Care Pros are trained to recognize early warning signs — changes in balance, cognition, appetite or daily function — and to communicate those observations to families and the healthcare team promptly.
Training for hospital-to-home transitions includes high-acuity skills such as hoyer lifts, sit-to-stands, transfers, bed baths and catheter care, ensuring the Care Pro is prepared for whatever the client's discharge needs require.
