Your Role as an LPN / Client Care Advisor

As a Licensed Practical Nurse (LPN) at Home Instead, you will work closely with the office team to support quality client care, promote continuity of services, and provide clinical support to caregivers and clients. Responsibilities include:

  • Assist with client assessments and ongoing quality assurance visits.

  • Perform medication fills and support medication management.

  • Assist with developing, reviewing, and updating individualized care plans.

  • Monitor changes in client condition and communicate updates to the Registered Nurse, caregivers, families, and office staff.

  • Provide caregiver education and reinforce clinical skills and best practices.

  • Serve as a clinical resource for Care Professionals and office team members.

  • Ensure accurate, timely, and compliant clinical documentation.

  • Support adherence to Home Instead policies, state regulations, and best practices.

  • Advocate for person-centered, safe, and compassionate in-home care.

Schedule

  • Full-time or Part-time

  • 3–5 days per week (24–40 hours), with flexibility based on client needs

What You Bring to Our Team

  • Current Licensed Practical Nurse (LPN) license in good standing.

  • Strong clinical assessment and medication management skills.

  • Excellent communication and organizational skills.

  • Ability to work independently while collaborating with a multidisciplinary team.

  • Passion for serving older adults and helping them remain safely at home.

  • Valid driver's license, reliable transportation, and current auto insurance.
