GUIDE Dementia Care Resource

The GUIDE Program is an incredible resource that could benefit your loved ones living with a dementia diagnosis!

Through the GUIDE Program, eligible Medicare patients with a dementia diagnosis can receive up to $2,500 per year for home care / respite care services (which is equivalent to about 21 visits from Home Instead caregivers in 4 to 8 hour shift increments).

What is GUIDE and how Home Instead can help:

Medicare:
  • GUIDE is a program that stands for Guiding and Improved Dementia Experience. It aims to improve the quality of life for people living with dementia and their caregivers.
Services:
  • The program is for education and support for patients and their families. It is aimed to improve quality of life and keep them safe in their homes for as long as possible. Those approved receive respite care visits** from Home Instead caregivers.
    **Transportation is not included in these respite care services
Enrollment:
  • Visit a doctor or care team participating in GUIDE to get an assessment of health needs. That doctor or care team will then submit a form for approval. It's that easy!

Maintaining GUIDE Benefits:
  • Participation requires just one in-person visit per year and a monthly phone check-in with your GUIDE doctor or care team.
  • As long as the patient remains eligible, they're likely to be renewed each year in July.
