Position Summary
The Quality Assurance Coordinator is responsible for overseeing and maintaining the quality and compliance of all home care services. This role ensures that all patient care meets or exceeds regulatory standards and internal policies, and that documentation accurately reflects the care provided. The coordinator works closely with clinical and administrative teams to drive continuous improvement in patient outcomes, staff performance, and operational excellence.
Key Responsibilities
Conduct regular chart and documentation audits for accuracy, completeness, and regulatory compliance (state, federal, Medicare/Medicaid).
Review OASIS assessments, care plans, visit notes, and discharge summaries for quality assurance.
Monitor quality metrics, patient satisfaction, and incident trends; prepare and present QA/QI reports to leadership.
Coordinate and participate in Quality Improvement (QI) activities to enhance care outcomes and service delivery.
Provide feedback, education, and training to clinical staff on documentation, compliance, and quality standards.
Investigate and document patient complaints, adverse events, and follow up on corrective actions.
Stay up to date on regulatory changes and ensure agency policies are aligned with current standards.
Support the development and implementation of policies and procedures to improve overall quality and compliance.
Qualifications
Education: Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred; equivalent experience in healthcare quality assurance accepted.
Experience: Minimum 2–3 years in home health, home care, hospice, or healthcare quality/compliance.
Knowledge of: State and federal home care regulations, OASIS documentation, HIPAA, infection control, and accreditation standards (e.g., Joint Commission, CHAP).
Skills:
Strong attention to detail and analytical ability.
Excellent written and verbal communication skills.
Proficiency in EMR systems (Homecare Homebase, AlayaCare, MatrixCare, or similar).
Ability to work independently and collaboratively across teams.
