What You’ll Do:

? Ensure documentation and care plans meet all compliance requirements

  • Oversee and maintain the quality, accuracy, and compliance of all client care documentation and service delivery

  • Develop, review, and update care plans to reflect clients’ evolving needs

  • Provide ongoing education and support to caregivers and families to improve understanding and care outcomes

  • Monitor client satisfaction, engagement, and retention, ensuring issues are addressed quickly and compassionately

  • Conduct quality assurance audits, identify trends, and report findings to leadership

  • Support continuous improvement initiatives that enhance the client experience and caregiver performance

  • Partner with the team to keep clients safely on service and at home

Position Summary

The Client Care Advisor is responsible for overseeing and maintaining the quality and compliance of all home care services. This role ensures that all patient care meets or exceeds standards and internal policies, and that documentation accurately reflects the care provided. The coordinator works closely with caregiver and administrative teams to drive continuous improvement in patient outcomes, staff performance, and operational excellence.


Key Responsibilities

  • Conduct regular chart and documentation audits for accuracy, completeness, and regulatory compliance.

  • Review assessments, care plans, visit notes, and discharge summaries for quality assurance.

  • Monitor quality metrics, patient satisfaction, and incident trends; prepare and present QA/QI reports to leadership.

  • Coordinate and participate in Quality Improvement (QI) activities to enhance care outcomes and service delivery.

  • Provide feedback, education, and training to staff on documentation, compliance, and quality standards.

  • Ensure care is accurate, provide activities,

  • Help monitor the care our clients receive in the home - support families with resources

  • Support the development and implementation of policies and procedures to improve overall quality and compliance.


Qualifications

  • Education: Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred; equivalent experience in healthcare quality assurance accepted.

  • Experience: Minimum 2–3 years in home health, home care, hospice, or healthcare quality/compliance.

  • Knowledge of: State and federal home care regulations, OASIS documentation, HIPAA, infection control, and accreditation standards (e.g., Joint Commission, CHAP).

  • Skills:

    • Strong attention to detail and analytical ability.

    • Excellent written and verbal communication skills.

    • Proficiency in EMR systems (Homecare Homebase, AlayaCare, MatrixCare, or similar).

    • Ability to work independently and collaboratively across teams.
