Client Care Coordinator

Job Details

We are looking for an individual to join our team as a Client Care Coordinator in our office. The Client Care Coordinator is expected to perform a variety of duties that relate to client care, with the primary task involving a structured process for the purposes of ensuring the highest quality experience for our families and maintaining ongoing awareness of client needs. They continuously evaluate each client's care plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. The Client Care Coordinator may also respond to client inquiries to our office, conduct care consultations with potential clients and family members and perform client/CAREGiverSM introductions.

Primary Responsibilities Include:                                                                             

  • Work with other staff towards a goal of high client and CAREGiver satisfaction.
  • Identify and implement ways to be proactive instead of reactive.
  • Make ongoing phone calls to and visits with clients as part of a quality assurance plan.
  • Work with other team members to coordinate various aspects of a client’s care.
  • Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures and business ethics codes and ensure that they are communicated and implemented within the team.
Qualifications: 
  • College degree preferred
  • Two years experience in home care or senior-related industry required, an equivalent combination of education and work experience may be considered
  • One year experience in health care and working with a senior population most desirable
  • Must possess a valid driver’s license
  • Must have computer skills and be proficient in Word and Excel
  • Must have the availability to work evenings or weekends as required
  • Must have the ability to 
    • demonstrate excellent oral and written communication skills and the ability to listen effectively
    • work independently, maintain confidentiality of information and meet deadlines
    • the ability to work as part of a team
    • use effective interpersonal skills as well as sound judgment and good decision-making skills

Job Details

Our office of Home Instead Senior Care, serving families throughout Northeastern, CT, has an opening for a Human Resources position. In our office, this position is referred to as a Recruitment Coordinator though the role encompasses a number of human resources responsibilities beyond recruitment. Most generally, the Recruitment Coordinator is expected to perform a variety of duties for the recruiting, hiring, training, onboarding and ongoing management of CAREGiversSM. The Recruitment Coordinator works together with the Engagement Coordinator to hire, support and retain a staff of CAREGivers to provide the highest quality service to our aging adult clients. In addition, the Recruitment Coordinator is responsible for all corrective action steps for the staff of CAREGivers.

Primary Responsibilities:                                                                             

  • Develop and implement active and passive recruitment strategies (e.g., web-based job search engines, social media, outreach, networking)
  • Schedule and conduct applicant interviews in an efficient and professional manner
  • Ensure the completion of all Home Instead Senior Care Standards with regard to the hiring process (e.g., reference checks, background checks, motor vehicle checks and drug screens)
  • Schedule and conduct CAREGiver orientation and work with Training Coordinator to schedule all training required to meet Home Instead® Standards and additional optional training
  • Schedule and conduct CAREGiver annual reviews and all supervision including regular performance conversations, accountabilities and problem resolution
  • Work in partnership with the Scheduling Department to coordinate CAREGiver schedules with an emphasis on creating high quality matches and extraordinary relationships
  • Monitor, mediate, and log CAREGiver activity (coaching, corrective action, problem resolution, conflict resolution, etc.)
  • Evaluate and update all orientation and training materials as needed
  • Plan and successfully execute all CAREGiver meetings
  • Respond to DOL employment paperwork
  • Monitor compliance for local and federal labor and safety laws including EOE, ADA, FMLA and OSHA

Knowledge, Skills and Abilities:

  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures

After training CAREGivers will feel confident going into the client’s home helping them achieve success with their emotional, physical and mental wellbeing. 

Requirements to be a CAREGiver are:

  • Ability to treat and care for seniors and their property with dignity and respect
  • Ability to communicate with clients in a friendly and congenial manner
  • Demonstrated reliability and trustworthiness
  • Complete a criminal background check, DMV check, and drug screen.

  • Must be available to work a schedule of 15-40 hours per week.

· Light housekeeping tasks and meal preparation

· Medication and appointment reminders

· Care for aging adults who need added engagement and/or assistance with accomplishing their basic daily activities.

*No previous experience necessary - skills training will be provided by Home Instead Senior Care*

Requirements:

· Complete a criminal background check, motor vehicle record check, and drug screen

· Possess a valid driver’s license and valid auto insurance (required if driving clients)

· Ability to treat and care for seniors and their property with dignity and respect

· Ability to communicate with clients in a friendly manner

On-Call Weekend CAREGiver

Are you someone who has great flexibility on your weekends (Friday evening through Sunday evening)? Enjoys working with a variety of people whose needs may vary? Likes knowing that you are making a tremendous difference in the lives of seniors? Then you might just be the person we're looking for! Home Instead Senior Care is looking for a caring and compassionate Weekend On-Call CAREGiver  to become a part of our team and join our mission of enhancing the lives of aging adults throughout our community.

Hours:                     Friday beginning 5:30 pm through Monday 8:00 am; days, evenings, overnights possible within Windham and Tolland Counties.
                                Must be willing to work with both male and female clients; must be able to work in a home with pets (i.e. dogs, cats).

Compensation:       $100.00 stipend per weekend AND hourly compensation paid for all service hours completed with clients

As an On-Call Weekend CAREGiver, it is highly likely that you will be called to cover short-notice shifts and/or to conduct CAREGiver introductions.If you decline the offer of a shift, you will forfeit the $75.00 stipend.

Duties may include, but are not limited to:

  • Light housekeeping tasks and meal preparation
  • Medication and appointment reminders
  • Assistance with grooming and bathing
  • Assistance with toileting and incontinence care
  • Assistance with transfers and ambulation
  • Companionship and client engagement

Requirements to be an On-Call CAREGiver:

  • A minimum of 2 years personal care experience required; CNA preferred.
  • Must complete mandatory in-house Personal Care and Alzheimer’s/Dementia training courses.
  • Ability to treat and care for seniors and their property with dignity and respect
  • Ability to communicate with clients in a friendly and congenial manner
  • Complete a criminal background check, DMV check and drug screen
  • Possess a valid driver’s license, reliable transportation and valid auto insurance
