Client Care Services

Job Details

Objective:
 

Home Instead Senior Care is looking for an outgoing, detail oriented individual as our Client Care Coordinator (CCC) in the organization. This position is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiver introductions and quality assurance visits with existing clients. He/she will use the consultative sales approach to determine each individual client's needs to provide solutions and create a tailored service plan. He/she continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours. We are located at 11201 Richmond Avenue in Houston, Tx 77082.

Primary Responsibilities:

  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process.
  • Answer each incoming call in a friendly, professional, and knowledgeable manner and distribute incoming calls to the appropriate staff members
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families.
  • Work with other team members to coordinate various aspects of a client's care.
  • Conduct client/CAREGiver introductions with every new client and with every new CAREGiver.
  • Create and maintain client and responsible party records documenting all quality assurance meetings.
  • Plan and execute schedule that ensures each client quality assurance visits as listed in the Plan of Care.
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
  • Demonstrate open and effective communication with clients, family members, the entire team, colleagues and CAREGivers, and referral providers/care providers.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • MUST be a team player and adaptable to daily needs. Must Have Experience in Clear Care.
  • Maintain regular attendance at the office to execute job responsibilities while utilizing all phases of Clear Care
  • Build relationships with potential referral sources according to the marketing plan.
  • Create and execute networking activities to support the marketing plan.
  • Able to identify, troubleshoot, and maintain excellent quality assurance for the organization.
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team.

Secondary Responsibilities:

  • Participate as needed in all CAREGiver meetings
  • Conduct Family Education sessions as needed
  • Perform any and all other functions deemed necessary

 

Education/Experience Requirements:

  • College degree preferred, but not required
  • One year experience in home care, health care or senior-related industry required; an equivalent combination of education and work experience may be considered
  • Must possess a valid driver's license

 

Knowledge, Skills and Abilities:

  • Must have an understanding of and uphold the policies and procedures established by Home Instead Senior Care and state regulations.
  • Must have good decision-making skills & work independently.
    Must demonstrate excellent oral and written communication skills and the ability to listen effectively

Recruitment and Engagement Coordinator

Objective:
The Recruitment and Engagement Coordinator is expected to perform a variety of duties for the recruitment and retention of CAREGivers. The Recruitment and Engagement Coordinator's primary responsibilities include but are not limited to recruiting, screening, hiring, training and engaging a staff of CAREGivers in order to provide the highest quality service to clients.

 

Primary Responsibilities:

  • Screen and review CAREGivers' applications and answer job inquiries.
  • Develop and implement new recruitment strategies online and within the community.
  • Schedule and conduct applicant interviews in an efficient and professional manner.
  • Conduct reference checks, criminal background and motor vehicle check and drug screens on all CAREGivers.
  • Create and maintain all employment records including but not limited to Form I-9, W-4 form and all other employment related documents.
  • Schedule and conduct CAREGiver orientation and all training required to meet Home Instead® Standards.
  • Develop engagement strategies that utilize the 5 CAREGiver needs to promote CAREGiver retention and satisfaction.
  • Schedule and conduct CAREGiver annual reviews and all supervision including regular performance conversations, accountabilities and problem resolution.
  • Work in partnership with the Scheduling Department to coordinate CAREGiver schedules with an emphasis on creating high quality matches and extraordinary relationships.
  • Monitor, mediate, and log all client and CAREGiver activity utilizing the software system.
  • Prepare and publish the monthly newsletter within the deadline.
  • Evaluate and update all orientation and training materials as needed.
  • Plan and successfully execute all CAREGiver meetings.
  • Monitor compliance for local and federal labor and safety laws including EOE, ADA, FMLA, and OSHA.
  • Maintain punctual, regular attendance at the office to execute job responsibilities, including On-Call duties.
  • Demonstrate open and effective communication with colleagues, CAREGivers, clients, and family members.
  • Conduct Staffing, Service Inquiries and Care Consultations as needed following the established sales process.

Home Instead Senior Care provides a variety of non-medical services that allow seniors to remain in their home and meet the challenges of aging with dignity, care and compassion. CAREGiver duties include, but are not limited to:

  • Companionship and conversation
  • Personal care (bathing, dressing, incontinence care etc)
  • Light housekeeping and meal preparation
  • Laundry and Linens 
  • Medication and appointment reminders
  • Transportation ( appointments/errands)
  • Alzheimer’s Care
  • IN ADDITION PET FRIENDLY DOGS & CATS
*No previous experience necessary - skills training provided by Home Instead Senior Care

Requirements to be a CAREGiver:
  • Possess a valid driver’s license and valid auto insurance
  • Reliable vehicle for transportation 
  • Social security card
  • Complete a criminal background check, DMV check and drug screen

  • Ability to communicate with clients in a friendly and congenial manner
  • Complete a criminal background check, motor vehicle record check and drug screen
  • Possess a valid driver’s license and valid auto insurance (required if driving clients)
  • IN ADDITION PET FRIENDLY DOGS & CATS
