RN Client Care Manager
Choice Services, Inc d/b/a Home Instead Senior Care is looking for a caring and compassionate leader to become part of our team and join our mission of Enhancing the Lives of Aging Adults and their Families. The RN Client Care Manager is responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients, as well as the networking, marketing, and external sales.

  Objective:

  • Ensures the ongoing satisfaction, quality, and safety of existing clients through regular contact, in conjunction with designated quality assurance home visits for clients who may use medication administration and nursing delegated specialized services.
  • Establishes and monitors results-producing relationships with referring physicians, case managers, social workers, and other industry related organization.
  • The position requires the exercise of discretion and independent judgment regarding significant matters of quality assurances/clinical management/risk management and training of the CAREGiversSM for Home Instead Senior Care.
  • Clear understanding of RN regulations and responsibilities for Nebraska as related to nursing delegation to an unlicensed caregiver.
 
  • Primary Responsibilities:
  • Reflect the values of Choice Services, Inc d/b/a Home Instead Senior Care
  • Demonstrate open and effective communication with clients, family members, franchise managers, franchise owners, colleagues, CAREGivers, and referral providers/care providers.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures, performance objectives, workplace behaviors, and business ethics codes and ensure that they are implemented and communicated to all employees.
  • Participate in the development and management of the annual franchise operational plan and budget.
  • Achieve the annual operational plan’s revenues, gross profit, profitability and other operational goals by the stated milestones.
  • Provide leadership and management of the client services and business development departments which affect profitability and achievement of the operational goals.
  • Manage outcomes of department activities and direct changes that will drive achievement of operational goals.
  • Develop and maintain a structured, measurable and inclusive Quality Assurance program that builds trust with each client and CAREGiverSM.
  • Develop and maintain positive and professional relationships with referral sources in the community.
  • Conduct service inquiries as needed following the consultative sales process.
  • Conduct Care Consultations as needed following the consultative sales process
  • Perform on Call Coordination on rotating weekdays, weekends, and holidays.
  • Initiate and manage medication administration and/or nursing delegated specialized services to clients.
  • Maintain a current plan of care outlining the specific specialized services to be provided to the client. The plan of care must be updated at minimum every 60 days and documentation should be retained electronically or within the client’s file.
  • Conduct quality assurance home visits with new clients at minimum every 60 days for clients receiving medication administration or delegated specialized services.
  • Ensure proper maintenance of client journals: collect service records for office file and keep journal stocked with appropriate documentation forms.
  • Provide oversight, up to and including the instruction of initial and ongoing in-office and field training for CAREGivers.
  • Conduct ongoing in-office and field trainings for CAREGivers.
  • Conduct specialized services trainings with individual CAREGivers who are serving high needs clients receiving specialized services. The training must include a return demonstration performed by the CAREGiver and signed off/dated by the nurse. This documentation must be retained in the CAREGiver’s employment file.
  • Act as an expert adviser regarding the medication services and delegated specialized services training given to CAREGiver.
  • Demonstrate mastery of the following technical systems/processes/resources: (for example Office365 Operating System, Outlook Email, Clear-Care, Adobe Acrobat for Document Scanning, Email to Fax, Text Messaging, The Franchise Center, The Hub, and more).
  • Keep detailed documentation of all relevant phone and in-person contact with clients, client representatives, CAREGivers and others.
  • Review, create and keep updated the medication administration and specialized services records for clients receiving those services.
  • Attend reoccurring administrative meetings.
  • Travel throughout our 14 county service area.

        Secondary Responsibilities:

  • Maintain and establish strong relationships with neighboring franchise owners.
  • Maintain and establish strong relationships with the Home Instead, Inc. Business Performance team.
  • Perform any and all other functions and responsibilities deemed necessary for the successful operation of a Home Instead Senior Care franchise office.

    Education/Experience Requirements:

  • Maintain a current, non-restricted Registered Nursing License in Nebraska.
  • 2 to 5 years of nursing experience with nurse delegation and management of employees.
  • Geriatric nursing experience preferred
  • Adhere to the requirements of Home Instead, Inc. as outlined in the Operations Manual.
  • Assist with the medication and other comprehensive clinical services as outlined in Nebraska Administrative Rules.
  • Comfortably and confidently delegate nursing tasks as outlined in Nebraska Administration Rules.
  • Demonstrate clear understanding of the rules and regulations regarding delegating nursing tasks to both other licensed and non-licensed employees.
  • Must possess a valid driver’s license
 
  • Knowledge, Skills and Abilities:
  • Must have an understanding of and uphold the policies and procedures established by Choice Services, Inc., (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively

Client Care Coordinator 

Objective:

Choice Services, Inc. DBA Home Instead Senior Care is looking for a dynamic, energetic, and compassionate leader to become part of our team and join our mission of enhancing the lives of aging adults and their families. 

The Client Care Coordinator is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiverSM introductions, quality assurance visits with existing clients, and maintaining positive relationships with our referral provider network. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours.

This position is also expected to perform a variety of on-call duties. When providing coordination of on call services, it is expected that you work closely with the Service Coordination team in order to provide the highest quality service to clients.

Primary Responsibilities:

  • Reflect the core values of Choice Services, Inc. dba Home Instead Senior Care.
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
  • Work with other team members to coordinate various aspects of a client’s care.
  • Conduct or coordinate client/CAREGiver introductions with every new client and with every new CAREGiver and perform first shift follow up phone calls.
  • Create and maintain client and responsible party records documenting all quality assurance meetings.
  • Plan and execute a schedule that ensures each client has at minimum one quality assurance visit every three months. Higher volume clients will need to have a greater frequency of visits.
  • Develop and maintain positive and professional relationships with referral sources in the community.
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
  • Demonstrate open and effective communication with clients, family members, owner, managers, colleagues, CAREGivers, and referral providers/care providers.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures, performance standards, and business ethics codes and ensures that they are communicated and implemented within the team.
  • Participate in 100% of scheduled meetings with team members and Quarterly CAREGiver meetings.
  • Conduct training sessions to engage and educate the CAREGivers and community
  • Perform On Call Coordination on rotating weekends and holidays.

Education/Experience Requirements:

  • College degree
  • One year experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
  • Must possess a valid driver’s license

Business Development Manager


Choice Services, Inc. DBA Home Instead Senior Care is seeking a high energy individual, who is results focused and committed to the growth of our organization. Objective:

Develops and executes business development strategies. Establishes and monitors results-producing relationships with referring physicians, case managers, social workers and other industry related organizations. Educates team members and referral sources about service capabilities. Must understand the competitive Home Care environment and act as a key member behind the process of identifying, evaluating, building and implementing new business opportunities that achieve significant revenue and segment profit. Working under minimal supervision, this position is responsible for growing market share across industry - this is done through, qualifying opportunities, penetrating new accounts, expanding existing client relationships, or managing strategic partnerships.

Primary Responsibilities:

  • Reflect the core values of Choice Services, Inc. DBA Home Instead Senior Care
  • Demonstrate open and effective communication with clients, family members, franchise members, franchise owners, colleagues, CAREGivers and referral providers/care providers.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures, performance objectives, workplace behaviors, and business ethics codes and ensure that they are implemented and communicated to all employees.
  • Identify areas of opportunity to expand client care revenue and community relationships
  • Develop and implement sales strategies with Owner and Client Care Manager to increase business
  • Develop sales and marketing plans, detailing weekly and monthly activities focused on meeting or exceeding established goals in conjunction with franchise leadership
  • Conduct research on new and untapped referral sources
  • Collaborate with departments and resources to meet the needs of prospective and active business clients
  • Effectively present Home Instead Senior Care marketing materials and services to referral providers, including 1:1 meetings and presentations in group settings
  • Plan, arrange and conduct formal group presentations to referral providers
  • Maintain a pulse on the strategic position of the company within the market
  • Work independently and collaboratively to meet and exceed monthly, quarterly, and annual sales targets
  • Develop a database of qualified leads from referrals through face-to-face cold calling on referral providers, email, telephone and Web contacts
  • Participate in and contribute to the development of educational programs offered to clients, prospects, and co-workers
  • Pursue a program of self-development through the use of selected reading, seminars, and participation in continuing education
  • Develop and maintain knowledge of Home Instead Senior Care brand and healthcare industry
  • Perform On Call Coordination on rotating weekends and holidays.

Secondary Responsibilities:

  • Conduct introductions, care consultations and quality assurance visits as deemed necessary
  • Field new client inquires over the phone in a knowledgeable manner and enter information into the operating system
  • Perform any and all other functions and responsibilities deemed necessary

Education/Experience Requirements:

  • Results-oriented individual with a track record of substantial professional sales accomplishments
  • Existing knowledge of and relationships with healthcare facilities, agencies, and community groups
  • Minimum of two years of experience in sales or marketing capacities, with at least one year in a health care setting or company is preferred
  • Have existing relationships with home health care referral sources
  • Proven record of demonstrating the drive and creativity to increase client base
  • College degree or equivalent work experience
  • Must possess a valid driver’s license
  • Must be able to work in the field, and be insurable

Knowledge, Skills and Abilities:

  • Must have an understanding of and uphold the policies and procedures established by Choice Services, Inc., (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must express hgh level of creativity, flexibility, and decisiveness
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
  • Must demonstrate knowledge of the senior care industry
  • Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work
  • Must have the ability to establish good working relationships and effectively communicate with the franchise owner, office colleagues, CAREGivers, clients, referral providers and community members
