CNA Client Care Coordinator

Objective:

Choice Services, Inc. DBA Home Instead Senior Care is looking for a dynamic, energetic, and compassionate leader to become part of our team and join our mission of enhancing the lives of aging adults and their families. 

The CNA Client Care Coordinator is expected to perform a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiverSM introductions, quality assurance visits with existing clients, and maintaining positive relationships with our referral provider network. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the plan through a series of ongoing communications and visits to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours.

This position is also expected to perform a variety of on-call duties. When providing coordination of on call services, it is expected that you work closely with the Service Coordination team in order to provide the highest quality service to clients.

Primary Responsibilities:

  • Reflect the core values of Choice Services, Inc. dba Home Instead Senior Care.
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
  • Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
  • Work with other team members to coordinate various aspects of a client’s care.
  • Conduct or coordinate client/CAREGiver introductions with every new client and with every new CAREGiver and perform first shift follow up phone calls.
  • Create and maintain client and responsible party records documenting all quality assurance meetings.
  • Plan and execute a schedule that ensures each client has at minimum one quality assurance visit every three months. Higher volume clients will need to have a greater frequency of visits.
  • Develop and maintain positive and professional relationships with referral sources in the community.
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
  • Demonstrate open and effective communication with clients, family members, owner, managers, colleagues, CAREGivers, and referral providers/care providers.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Maintain regular attendance at the office to execute job responsibilities.
  • Adhere to all company policies, procedures, performance standards, and business ethics codes and ensures that they are communicated and implemented within the team.
  • Participate in 100% of scheduled meetings with team members and Quarterly CAREGiver meetings.
  • Conduct training sessions to engage and educate the CAREGivers and community
  • Perform On Call Coordination on rotating weekends and holidays.

Education/Experience Requirements:

  • CNA Certification or healthcare related degree
  • One year experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
  • Must possess a valid driver’s license

  • Must have an understanding of and uphold the policies and procedures established by Choice Services, Inc., (d.b.a. an independently owned and operated Home Instead Senior Care franchise)
  • Must demonstrate excellent oral and written communication skills and the ability to listen effectively
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines
  • Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making skills
  • Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures
  • Must demonstrate knowledge of the senior care industry
  • Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work
  • Must have the ability to establish good working relationships with the franchise owner, office colleagues, CAREGivers and the community
  • Must have the ability to present a professional appearance and demeanor

Full Time Caregiver

Job Details

The Full Time CAREGiver is expected to provide a variety of non-medical services that allow seniors to remain in their homes.  These services are meant to enhance the quality of life for seniors. A Home Instead CAREGiver will help clients and their families meet the challenges of aging with dignity, pride, care and compassion.  The Full Time CAREGiver is also expected to be on call for shift work as scheduled, perform client to CAREGiver introductions and serve a role model and mentor to other CAREGivers.  The Full Time CAREGiver will also at times be assigned to administrative tasks, such as providing clerical and office support.   

 

Essential Job Requirements:

  • 7 AM to 10 PM availability with every other weekend off
  • Work a full-time schedule
  • Work every other weekend
  • Reliable transportation
  • Travel throughout the entirety of our 14-county coverage area
  • Proficient utilization of technology system including a Mobile App.
  • Perform on call shifts as scheduled
  • Perform CAREGiver to client introductions as scheduled
  • Ability to work 3 holidays per year
  • Ability to attend Quarterly CAREGiver meetings
  • Ability to communicate on a weekly basis with immediate supervisor

Position Compensation and Benefits:

  • Hourly wage of $12.50 per hour, eligible for wage increase following a 6 month performance review
  • Every other weekend off
  • PTO of 40 hours per year

Education/Experience Requirements:
  • College degree or equivalent work experience
  • Two to four years of related business experience or an equivalent combination of education and work experience may be considered
  • Must possess a valid driver’s license
  • Experience in the healthcare industry is preferred.

Supervisory Responsibilities:
