GUIDE Program for Dementia Care

Providing care for a loved one living with dementia can be a difficult and draining process for many family caregivers.  Home Instead is committed to providing family caregivers with the support and resources they need to not only care for their loved one, but also to ensure they do not fall victim to caregiver burnout.​  

In home care from Home Instead ensures that routines can be maintained while giving family caregivers the break they need.  As part of our commitment to supporting families living with dementia, Home Instead in Belton, TX is proud to offer dementia care services through the GUIDE Program for dementia care. 

What is the GUIDE Program?

Created in 2024 by the Centers for Medicare & Medicaid Services, the GUIDE (Guiding an Improved Dementia Experience) Program provides care support to those living with a dementia diagnosis and their family caregivers.  GUIDE benefits are available to Medicare Part A and B beneficiaries who have been diagnosed with a form of dementia and meet specific medical criteria.

Goals & Intended Benefits of the GUIDE Program include:

  • Improve quality of life for people with dementia through more proactive, holistic care
  • Reduce caregiver burden by offering education, support, respite, and 24/7 access
  • Reduce unnecessary hospitalizations, ER visits, re-admissions, post-acute utilization, and delay or prevent long-term nursing home placement
  • Cost savings for Medicare and Medicaid over time by reducing higher-cost services and institutional care
  • Standardize dementia care practices by setting benchmarks and expectations for care delivery
  • Address health equity in underserved areas to support care in disadvantaged populations.

Home Care Services Available through the GUIDE Program include:

  • Care planning, assessment, and coordination — We will create an individualized, person-centered care plan tailored to the individual’s dementia stage and care needs and coordinate with the person’s care team, including family, physicians and home health agency.
  • Caregiver education and support — Home Instead provides family caregivers with education and care resources to make family caregiving easier.
  • 24/7 care and supportWe will develop a schedule that fits the needs of the individual and their family caregiver to ensure the most effective use of GUIDE benefits with care available around the clock, 7 days a week.
  • Respite care services — In-home respite care ensures the individual living with dementia can stay in a routine and location that is familiar while providing family caregivers the break they need.  The GUIDE program provides a capped amount of respite care hours each year to beneficiaries.
