RN - Home Care Quality Assurance

Home Instead is seeking a part-time, compassionate, detail-oriented Registered Nurse to serve as a Quality Assurance Specialist focused on delivering exceptional care coordination for our veteran clients. This role combines clinical expertise with relationship building to ensure our veterans receive the highest standard of personalized home care services.
This position requires approximately 60 hours per month with a flexible schedule to accommodate client visit needs, including occasional evenings or weekends.
Key Responsibilities:
Clinical Assessment & Quality Assurance:
- Conduct in-home QA visits with veteran clients, assessing physical, cognitive, and psychosocial needs
- Perform Care Consultations and create individualized care plans addressing service-connected conditions and veteran-specific needs
- Monitor care delivery standards, adjust plans as conditions evolve, and ensure clinical appropriateness of services
Documentation & Compliance:
- Maintain HIPAA-compliant clinical records, including assessments, care plans, and QA findings
- Ensure all services meet company standards, state regulations, and best practices in veteran care
Collaboration & Coordination:
- Partner with VA healthcare providers, families, and our care team to ensure seamless service delivery
- Serve as primary point of contact and trusted advocate for veteran clients and their families
Licensure & Education:
- Current, unrestricted RN license in [state]
- Associate's or Bachelor's degree in Nursing
Experience:
- Minimum 2 years of clinical nursing experience; home health, geriatrics, or VA healthcare experience
- Experience working with veterans, older adults, and individuals with chronic/complex conditions
- Background in care coordination, case management, or quality assurance is a plus
Other Requirements:
- Valid driver's license and reliable transportation for client home visits
- Flexibility to work occasional evenings or weekends as needed
- Proficient computer skills, including Microsoft Office and electronic documentation systems
