Position Summary

The Registered Nurse (RN) – Medicaid & Waiver Programs Support plays a key role in coordinating and supporting home care services funded through Medicaid Advantage and other waiver programs in Oklahoma. This position works closely with the Administrator/RN Supervisor to ensure compliance, high-quality care delivery, and successful coordination of services for clients receiving personal care assistance.

The RN is responsible for completing clinical assessments, supporting care plan development, monitoring ongoing services, and ensuring all regulatory and payer requirements are met.

Key Responsibilities

Clinical & Client Care

  • Conduct initial and ongoing nursing assessments for clients receiving personal care services

  • Develop, implement, and update individualized care plans in collaboration with clients, families, and interdisciplinary teams

  • Monitor client condition and ensure services meet medical necessity and payer requirements

  • Provide clinical oversight to caregivers to ensure safe, effective care delivery

  • Address changes in client condition and coordinate appropriate interventions

Medicaid Advantage & Waiver Program Support

  • Assist with coordination of services under Medicaid Advantage and applicable Oklahoma waiver programs (e.g., ADvantage Waiver, personal care services, VA)

  • Ensure compliance with Oklahoma Health Care Authority guidelines and regulations

  • Support eligibility processes, service authorizations, and documentation requirements

  • Maintain accurate, audit-ready records for all Medicaid-funded services

  • Communicate with case managers, MCOs, and state agencies as needed

Care Coordination & Collaboration

  • Serve as a liaison between clients, families, caregivers, and referral sources

  • Collaborate with the Administrator/RN Supervisor to ensure continuity of care and service excellence

  • Assist in onboarding new clients, including assessments and service initiation

  • Participate in case conferences and interdisciplinary team meetings

Quality Assurance & Compliance

  • Ensure all services meet state, federal, and company standards

  • Participate in quality assurance activities, including chart audits and compliance reviews

  • Identify areas for improvement and assist in implementing corrective actions

  • Maintain knowledge of current Medicaid policies and waiver program updates

Training & Support

  • Provide guidance and education to caregivers on client-specific care needs

  • Assist in skills validation and competency checks as needed

  • Support staff in understanding documentation and compliance expectations

Qualifications

Education & Licensure

  • Active Registered Nurse (RN) license in the State of Oklahoma

  • Graduate of an accredited nursing program

Experience

  • Minimum of 1–2 years of nursing experience (home care or community-based care preferred)

  • Experience with Medicaid programs, waiver services, or long-term care preferred

  • Familiarity with personal care services and non-medical home care strongly preferred
