Aging Well Program

​A wellness program built into our services that allows Home Instead to provide our clients with a proactive approach to aging well and staying in their home. Aging Well focuses
on quality of life and healthy living.

Setting Goals:

Clients and clinical nurse case managers collaboratively set goals across 8 points of wellness and develop strategies to strive toward by the next Quality Assurance visit within six months.

Nurse Advocacy & Resources:

Helping clients find and connect to home health community support & resources, as well as providing vetted referrals to help mitigate challenges.

Monitoring Progress:

Our clinical nurse case managers use a unique assessment & score tracking system at each Quality Assurance visit to monitor progress of every individual wellness area over time.
