We are seeking a compassionate and detail-oriented Part-Time Registered Nurse (RN) to join our team. This role involves local client home visits and care coordination.

Job Duties:

• Conduct in-home client visits

• Perform client reassessments

• Update and maintain client care plans

• Complete TB risk questionnaires and required documentation

• Communicate effectively with clients and the care team

Qualifications:

• Active Registered Nurse (RN) license

• Experience with client assessments preferred

• Strong written and verbal communication skills

• Must be authorized to work in the United States

• Reliable transportation for local visits
