Your Key Responsibilities:

  • Organize Client Visits: Schedule and complete visits with clients in their home to ensure their level of care is adequate, the number of service hours are appropriate, and that there is a good client and caregiver match

  • Support Clients: Address any client escalations where additional support and caregiver training is required

  • Perform Intros: Facilitate the initial meeting of the caregiver and client during their first shift together. Ensure important client information is shared with the caregiver, so that she can service the client well

  • Advocate for Client Authorizations: Assist with the retrieval and management of client authorizations as needed

  • Drive Results: Identify client health changes to advocate for additional hours of service to ensure proper care. Support clients through communication with care coordinators, service coordinators, case managers, and family members.

Secondary Responsibilities:

  • Client/Caregiver On-Call: Manage the monthly on-call schedule, oversee those who are taking on-call, and assist with issues as needed

  • Ongoing Initiatives: Look for areas of continuous improvement in existing processes to better support clients and their needs while building relationships to further grow the business by getting more clients and service hours.

What You Need to Succeed:

  • Educational Background: Must be a CNA with at least two years of home health experience AND have at least one year of recent supervisory experience

  • Consistency: Must be available to work full-time in the office, including evening and weekend on-call rotation

  • Engagement: Must have a passion to care for others by engaging with them to ensure they are getting the care they need and deserve

  • Embrace Technology: Must be extremely proficient in Microsoft Outlook, Microsoft Teams, and Microsoft Excel, and able to create and generate reporting to track key metrics. Must be proficient at understanding the use of online platforms and applications generally

  • Desire to Learn: Must be energetic about continual learning and embracing change in the home care landscape

  • Driving Requirement: Must possess a valid driver’s license and car insurance

  • Skills for Success: Strong communication, organizational skills, and professionalism
