The Home Care Community Liaison & Outreach Specialist is a sales-focused, community-facing role responsible for driving business growth through strategic outreach, relationship-building, and lead generation. This role requires someone who is confident making decisions about where to invest their time—focusing on relationships and opportunities that result in measurable referrals and new business.

We’re especially interested in candidates who are outgoing, community-minded, and naturally skilled in relationship-building. Experience in home care, healthcare marketing, senior services, or community engagement is a strong plus—but we’re open to training the right individual who brings warmth, drive, and professionalism to the role.

Primary Responsibilities:

  • Reflect the values of QC Senior Care Inc., an independently owned and operated Home Instead® franchise.
  • Lead external sales and outreach efforts to develop and manage relationships with high-potential referral sources such as hospitals, discharge planners, social workers, home health agencies, hospice providers, physicians’ offices, and other professionals in the aging services network.
  • Generate qualified client inquiries through consistent, high-impact outreach, including cold calls, scheduled visits, follow-ups, and relationship management.
  • Make strategic decisions about how to prioritize leads, referral partners, and outreach opportunities based on potential to generate inquiries—discerning which activities are worth your time.
  • Create and maintain a productive outreach schedule that balances nurturing high-performing relationships with seeking out new sources of referrals.
  • Confidently conduct 1:1 meetings and group presentations that effectively position Home Instead as the trusted resource for in-home care.
  • Use a consultative sales approach to uncover needs, offer solutions, and close new client opportunities.
  • Conduct care inquiries and in-home consultations with prospective clients and families when needed.
  • Attend and represent Home Instead at health fairs, senior expos, and networking functions—with clear referral-generation goals in mind.
  • Track and document all outreach activities, referral follow-ups, and inquiry outcomes using CRM software (Salesforce experience a plus).
  • Collaborate with leadership on developing and executing a sales and marketing plan aligned with revenue and inquiry goals.
  • Maintain open communication with the internal team to ensure new clients are smoothly transitioned into care.

Secondary Responsibilities:

  • Assist with CAREGiverSM and client introductions when needed.
  • Support follow-up efforts and quality assurance visits for newly referred clients.
  • Contribute to PR and educational efforts to increase community awareness and support brand visibility.

Key Metrics:

  • Complete 50 outreach contacts per week/month (calls, visits, emails).
  • Generate 20 qualified inquiries per month from outreach activity.
  • Convert 80% of inquiries to active clients per month.
  • Host or participate in 2 community events monthly

Education & Experience Requirements:

  • Bachelor’s degree preferred or equivalent experience in sales, marketing, communications, or healthcare.
  • Previous experience in outreach, business development, or relationship-based sales preferred.
  • Experience in home care, healthcare, or senior services is a plus.
  • Valid driver’s license and reliable transportation required.
  • Must be comfortable traveling throughout the local territory and working some evenings/weekends for events.

  • Proficient with computers, including Microsoft Word, Excel, and CRM tools. Salesforce experience is a major plus.
  • Detail-oriented and organized, with strong follow-up and documentation skills.
  • Passionate about helping others and promoting high-quality senior care.
  • Ability to work flexible hours, including some evenings or weekends.
