Position Summary
The Key Player – TCM Case Management & ARChoices Waiver Support provides essential administrative and coordination support to the RN Medicaid Client Care Advisor, with a primary focus on Targeted Case Management (TCM) and ARChoices Waiver clients. This role ensures timely follow-up, accurate documentation, and consistent communication with clients, families, and care teams.
The Key Player is responsible for managing day-to-day case management tasks, tracking client status, and ensuring all TCM and ARChoices requirements are met in compliance with Medicaid guidelines. This position plays a critical role in maintaining continuity of care and supporting client satisfaction.
Essential Duties and Responsibilities
TCM Case Management Support
- Assist with daily Targeted Case Management (TCM) activities for Medicaid clients
- Track and maintain TCM schedules, including monthly and quarterly follow-ups
- Complete routine client follow-up calls to assess needs, satisfaction, and service effectiveness
- Document all TCM contacts and activities accurately and timely
- Ensure TCM documentation meets compliance and billing requirements
- Prepare TCM documentation for RN review and approval
- Assist with TCM invoice tracking and submission processes
ARChoices Waiver Coordination
- Serve as a primary support contact for ARChoices Waiver clients and families
- Track ARChoices applications, renewals, and eligibility status
- Conduct consistent follow-ups on pending ARChoices applications and services
- Assist with gathering and submitting required ARChoices documentation
- Maintain organized records of all waiver-related communication and status updates
- Coordinate with the RN Supervisor to ensure timely completion of required assessments
Client Follow-Up & Care Coordination
- Conduct scheduled follow-up calls with ARChoices and Medicaid clients
- Identify changes in client condition, needs, or satisfaction and escalate to RN as needed
- Support coordination of services between clients, caregivers, and internal staff
- Ensure clients remain engaged and supported throughout the duration of services
- Assist with scheduling nursing assessments, reassessments, and supervisory visits
Medicaid Program Support
- Assist with tracking Medicaid eligibility processes and documentation
- Support follow-up on submitted forms (including Form 618) and pending approvals
- Maintain accurate and up-to-date client records in WellSky or applicable systems
- Assist with intake documentation and onboarding for new Medicaid clients
Administrative & Operational Support
- Maintain detailed tracking systems for TCM tasks, ARChoices status, and follow-ups
- Ensure all client files are complete, organized, and audit-ready
- Communicate regularly with the RN Supervisor regarding client updates and priorities
- Assist with reports related to TCM activity, ARChoices status, and client outcomes
- Support compliance with all Medicaid, ARChoices, and company policies
Qualifications
Required
- Strong organizational and time management skills

- Ability to manage multiple clients, tasks, and deadlines simultaneously
- Strong communication and interpersonal skills
- Proficiency in basic computer systems and documentation platforms
Preferred
- Experience with Medicaid home care services
- Knowledge of ARChoices Waiver program
- Experience with Targeted Case Management (TCM)
- Familiarity with WellSky or similar healthcare software
- Experience in healthcare coordination, case management, or home care
