Client Care Coordinator, RN

Three Rivers Senior Services, Inc., DBA Home Instead is looking for a compassionate Registered Nurse (RN) to join our team as a part-time Client Care Coordinator in Mountain Home, AR. You'll serve families across north central Arkansas, as well as Southern Missouri, covering Taney and Stone Counties. Together, we'll deliver personalized, high-quality in-home care to aging adults and their families, with a special focus on ARChoices (Arkansas Medicaid waiver) and Veterans Affairs benefits.

Position Details
- Part-time (approximately 2-3 days per week with flexible scheduling, primarily Monday–Friday)
- Field-based/mobile role: You'll spend most of your time in the community, conducting in-home consultations and quality assurance visits. Each morning, you'll receive your schedule of visits (from our Mountain Home office or remotely), then head out to meet with clients in their homes for assessments, quality checks, and care plan updates—with minimal office time required.
- Company vehicle available for business use (including all travel between visits)
Key Responsibilities
- Conduct in-home consultations with potential clients and their families to understand their needs, explore solutions together, and create detailed, personalized care plans in accordance with state and federal guidelines
- Perform regular quality assurance home visits to ensure exceptional care, verify that care plans are accurate and current, and maintain thorough, precise records
- Update care plans as needed to reflect changing client needs and ensure ongoing compliance with ARChoices, Veterans Affairs, and all applicable regulations
- Develop and maintain expertise in ARChoices and Veterans Affairs programs, staying fully compliant through careful attention to regulatory guidelines
- Address client and family concerns promptly and thoughtfully, providing expert guidance to uphold our high standards of care
- Stay current on home care regulations and best practices, completing all tasks efficiently and on schedule
- Contribute to positive client outcomes and satisfaction, supporting business growth through excellent care delivery
Qualifications
- Active Arkansas RN license (required)
- Multistate license through the Nurse Licensure Compact (NLC) highly preferred—both Arkansas and Missouri are compact states, enabling seamless practice across borders without additional licensure (a major advantage in our border region!)
- Preferred: Experience in home care, home health, case management, or regulatory/compliance (especially ARChoices, VA programs, or Medicaid waivers)
- Detail-oriented mindset with a proactive approach to completing responsibilities accurately and efficiently
- Strong organizational skills and ability to prioritize and manage multiple clients and tasks effectively (including independent travel and scheduling)
- Enthusiasm to become a specialist in ARChoices and Veterans Affairs home care
- Excellent verbal and written communication skills
- Independent worker who thrives in a collaborative, team-oriented environment
- Proficiency with computers and electronic records (for mobile documentation)
- Valid driver's license, reliable transportation, and auto insurance (required for extensive in-home travel)
- Compassionate, professional approach dedicated to senior care excellence
Compensation & Benefits
- $28–$34 per hour (commensurate with experience)
