GUIDE Program for Dementia Care
Caring for a loved one with dementia can be both deeply rewarding and challenging. We understand how important it is for families to have the right support. We are proud to be a provider for Medicare’s GUIDE Program (Guiding an Improved Dementia Experience), which is designed to provide additional resources and assistance to individuals living with dementia and their caregivers. Through GUIDE, you may be eligible for up to 72 hours of in-home care per year, at no cost to you.
What is the GUIDE Program?
Created in 2024 by the Centers for Medicare & Medicaid Services, the GUIDE (Guiding an Improved Dementia Experience) Program provides care support to those living with a dementia diagnosis and their family caregivers. GUIDE benefits are available to Medicare Part A and B beneficiaries who have been diagnosed with a form of dementia and meet specific medical criteria.

Through the GUIDE Program, those with dementia and their caregivers receive:
- CARE NAVIGATORS: A dedicated care navigator; Caregiver training, support and check-ins; Resources and referrals; Care and services coordination
- RESPITE CARE at NO COST to eligible participants: 72 hours, or $2,563/year towards in-home care
- MEDICAL OVERSIGHT: Telehealth accessibility; Medication review and management support; Comprehensive dementia planning; 24/7 access to 125+ specialty providers
Who is eligible?
- Diagnosed with dementia
- Traditional Medicare beneficiary
- Medicare is primary insurance
- Living at home or in an independent living community (not in an Assisted Living Facility, Memory Care or Nursing Home)
- Not participating in Medicare Advantage, PACE or Medicare Hospice benefits
- Has a family member caregiver or non-family unpaid caregiver
