Provide Companionship & Home Helper services which could include:

  • Companionship and conversation
  • Providing stabilization and assistance with walking
  • Preparing meals and cleaning up meal-related items
  • Providing medication reminders and appointment reminders
  • Performing light housekeeping tasks (dusting, vacuuming, making beds, changing linens, cleaning bathrooms, kitchens, etc.)
  • Washing and ironing laundry
  • Running errands
  • Accompanying clients to appointments
Essential Job Requirements:
Valid NY PCA Certification
Reliable Transportation Preferred
Experience with Dementia/Alzheimer’s care is a plus
Complete a criminal background check, DMV check and drug screen

Home Instead Senior Care is looking for an organized individual to join us in our mission to enhance the lives of aging adults and their families. 
The Client Care Coordinator role manages client satisfaction and retention and ensures high quality care through a series of ongoing communication and visits.

This is an ideal opportunity for an individual with a passion for senior care! Flexible schedule/hours available.

Primary Responsibilities:

  • Meet with potential clients and family members to discuss their needs and provide solutions.
  • Maintain ongoing communication and follow-up with potential clients and families.
  • Conduct client/CAREGiver introductions as needed.
  • Plan and execute a client quality assurance visit schedule and maintain accurate documentation.
  • Work with clients and their families on various issues and ensure they are getting resolution and receiving needed services.

Knowledge, Skills and Abilities:

  • Must demonstrate excellent oral and written communication skills, effective interpersonal and listening skills.
  • Must have the ability to work independently, maintain confidentiality of information and meet deadlines.
  • Must have computer skills.

Requirements:

  •  Experience in social work, senior care or healthcare is a plus.
  •  College degree preferred.
  •  Must possess a valid driver’s license & have access to reliable transportation.

Job Description
Home Instead Senior Care is seeking a Registered Nurse to join our team. As the Director of Patient Services you will serve as a key role for the organization by delivering quality care to patients, acting as an educator for the Home Instead team to improve efficiency with regards to patient services and overseeing compliance with NY State Department of Health regulations for a licensed home care agency.

Responsibilities:
• Oversee compliance with all state and federal regulations, laws, policies and procedures applicable to home health care services.
• Manage and evaluate the health services program to ensure quality of service and maintenance of state and agency policies and procedures.
• Perform consultative/supervisory home visits and assessments.
• Serve as a liaison between staff, patients, physicians, and other health care providers with regards to patient eligibility and suitability for home care services.
• Open new cases/ admission to Home Instead.

Qualifications:
• Registered nurse licensed to practice in New York State with supervisory or management experience in a home health care setting.
• Experience as a registered nurse and experience in home health care.
• Possesses current driver’s license and reliable automobile transportation.
