Job Description:

The Client Care Coordinator performs a variety of duties that relate to client care including, but not limited to, care consultations with potential clients and family members, client/CAREGiver introductions, care plan management, community outreach, and quality assurance. They use the consultative sales approach to determine each individual client’s needs to provide solutions and create a tailored service plan. They continuously evaluate the care plan through ongoing communications and visits with clients and their families to ensure high quality care, client satisfaction and retention as well as opportunities to increase service hours.

 

Primary Responsibilities:

  • Reflect the core values of Prescott Sanchez, Inc. d.b.a. an independently owned and operated Home Instead Senior Care franchise
  • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a care plan
  • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
  • Maintain ongoing communication and a consistent follow-ups with potential clients and families who have yet to commit to service
  • Work with other team members to coordinate various aspects of a client’s care
  • Conduct client/CAREGiver introductions with every new client and with every new CAREGiver
  • Create and maintain client and responsible party records documenting all quality assurance meetings
  • Plan and execute a schedule that ensures each client has at minimum one quality assurance visit per 90 days
  • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through the quality assurance program
  • Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed
  • Support community outreach and marketing initiatives to increase brand awareness
  • Manage Lead CAREGiver activities to support the quality assurance program
  • Conduct training classes on relevant senior topics with clients & CAREGivers
  • Demonstrate open and effective communication with the franchise owner, colleagues, CAREGivers, clients and family members
  • Maintain regular attendance to execute job responsibilities
  • Adhere to all company policies, procedures and business ethics codes and ensures that they are communicated and implemented within the team
  • Ensure Home Instead Senior Care standards are met and upheld

 

 

Qualifications:

Applicants with minimum 5 years experience in client care in a medical or non-medical setting. Knowledge of the senior care industry. Must have the ability to present a professional appearance and demeanor. Must be patient and congenial. Must have computer skills and be proficient in proficient in Microsoft Office Suite. Must be fluent in English, able to communicate information efficiently, possess strong work ethic, multi-task, able to follow through and complete assigned tasks. Must be a good listener, detail oriented, and the ability to prioritize tasks effectively. Applicant should be flexible and be available to work weekend as needed. Must have a valid drivers license, reliable car and car insurance. Bilingual, LVN, RN, MS in Gerontology, or health care experience is a plus.

Job Description:

The Marketing Specialist is responsible for achieving revenue growth within the Pasadena, CA and surrounding communities’ territory. Revenue is generated from a variety of sales and marketing activities which include researching and identifying new referral sources/opportunities as well as enhancing/maintaining relationships with our existing client referral sources. This position is responsible for transformational results in referral provider relationships, building on past successes and identifying new successes, and achieving results in new client inquiry generation through strong sales, marketing and networking opportunities.

Primary Responsibilities:

  • Reflect the core values of Prescott Sanchez, Inc. dba: Home Instead Senior Care, an independently owned and operated franchise.
  • Responsible for researching and developing client relationships and producing client referrals through our referral provider network; performs all phases of the sales process which includes preparation and effective first contacts, follow-up, diagnosis and solution-based alternatives of referral source/client needs, recommendation of appropriate services which can include local community resources above and beyond our service offerings, and maintaining and nurturing existing client referral sources.
  • Achieve inquiry generation objectives within Pasadena and surrounding communities with the primary goal of generating new business to increase overall market share. Target referral sources include hospitals, physicians, home health agencies, assisted living facilities, nursing homes, senior centers, and hospice services.
  • Through partnership with leadership, develops sales and marketing plans, detailing weekly and monthly activities focused on meeting or exceeding established goals and/or metrics.
  • Develop and maintain knowledge of Home Instead Senior Care brand.
  • Effectively presents Home Instead Senior Care marketing materials and services to referral providers, including face-to-face one-on-one meetings while also planning, arranging and conducting formal group presentations.
  • Maintain accurate sales, marketing and prospecting activities including sales calls, presentations, closed sales, and follow-up which includes the use of our user friendly, web-based software to maintain accurate records to maximize potential.
  • Develop and maintain a database of qualified leads from referral providers through in-person, email, telephone and web interactions.
  • May conduct prospective new client service inquiries and care consultations as needed using our consultative sales approach.
  • Participate in and contribute toward the development of educational programs offered to clients, prospects, and co-workers.
  • Adhere to all company policies, procedures and ethical codes of conduct and ensures that they are communicated and implemented within our team.
  • Ensure the utmost quality and standards are met representing the Home Instead Senior Care brand.

Minimum Requirements:

  • Applicants must have a minimum of five (5) years’ experience in sales or marketing.

  • Answer incoming calls in a friendly, professional and knowledgeable manner
  • Create and maintain client and CAREGiver schedules with an emphasis on creating high quality matches and the development of extraordinary relationships
  • Monitor, mediate, and log all client and CAREGiver activity utilizing the software system
  • Follow up with all client and CAREGiver issues to ensure their problems are resolved
  • Enter and maintain accurate client and CAREGiver records in the software system
  • Increase client loyalty to Home Instead Senior Care by utilizing the consultative sales process to better meet our current client needs
  • Follow up and communicate CAREGiver and client issues to ensure problems are resolved
  • Design, test and implement a responsive contingency plan that ensures 100% maintenance of Service Hours
  • Perform On-Call and Weekend Supervisor responsibilities as required by the business needs
  • Recognize and capture opportunities to increase service hours in scenarios to enhance and/or increase quality care
  • Field new client inquiries over the phone in a knowledgeable manner, enter the information into the software system and work with team to communicate and prepare for the Care Consultation
