Client Care Coordinator - RN

Home Instead is looking for a successful RN driven to ensure the best quality care for our clients, with a primary focus on state and federal programs such as ARChoices and Veterans Affairs. The Client Care Coordinator – RN must be compassionate and professional as they expertly assess client conditions and develop personalized plans for them. As the Case Manager for multiple clients, the successful candidate must be well-organized and work well in a team environment. While this position is based out of our Mountain Home office, we have satellite offices in Batesville and Harrison to help us better serve our clients throughout north central Arkansas. We are locally owned and managed while still having all the resources of the world’s largest and most reputable provider of home care services.

Responsibilities

  • Meet with potential clients and family members in their homes to discuss their needs and provide solutions.
  • Work well with other team members to coordinate various aspects of a client’s care.
  • Conduct client/Care Professional introductions.
  • Ensures each client receives regular quality assurance visits and maintains accurate records of those visits.
  • Ensures compliance with all state and federal programs.
  • Actively pursue opportunities to grow clientele and achieve growth goals.
  • Work with clients and their families to ensure positive client outcomes and resolutions to any concerns.

Qualifications

  • Arkansas-licensed RN
  • Willingness and desire to develop new business and grow the clientele
  • Willingness and desire to become the expert in applicable VA and ARChoices programs
  • Excellent written and verbal communication skills
  • Ability to work both independently and in a team environment
  • Ability to organize and prioritize work
  • Proficiency in computer and related technology skills
