Client Care Coordinator

Ocala

Join a fast growing team of individuals who care about enhancing the lives of Seniors in our community! We are looking for a Client Care Coordinator who would be able to support Clients and their families through this aging season. Must be CNA/HHA and sales experience preferred. You would be dealing directly with clients performing the following duties (not inclusive list):
  • Meet with potential clients and family members to discuss their needs and provide care solutions.
  • Work with other team members to coordinate various aspects of a client’s care.
  • Recognize and pursue opportunities to modify care plans to best support the ongoing needs of clients.
  • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the services needed.
  • Fill in for client shifts as needed.
Qualifications desired:
  • Outgoing personality who is a high level team player
  • Highly organized and able to handle consistent, necessary change
  • Ability to write and speak professionally
  • Desire to build trusting relationships with our clients and families
  • Have some experience with duties listed above
  • Be able to lift 25 pounds
  • Ability to speak calmly in stressful situations
  • Complete and pass a Level 2 nationwide criminal background check, DMV check and drug screen
  • Possess a valid driver’s license and auto insurance with you listed as a covered driver
