Home Instead® is looking for a successful sales-minded professional to join us in our mission to enhance the lives of aging adults and their families. This Care Manager role develops, manages and maximizes our client relationships throughout Northern Colorado.
Have excellent oral and written communication skills, the ability to listen effectively, work independently, meet deadlines, and maintain confidentiality.
Primary Responsibilities:
- Conduct Care Consultations with prospective clients and their families, conduct client and CAREGiver introductions, Quality Assurance/Supervisory Visits, and Safety Hazard Assessments, and follow up with appropriate correspondence.
- Enter and maintain accurate client and CAREGiver records pertaining to concerns, scheduling changes, etc. and follow up with internal staff, CAREGivers, and clients.
- Provide community resources and referrals to clients when needed and be an advocate for families when needed.
- Develop relationships with referral providers in the community and assist with community marketing events that would promote our service to the general public.
Knowledge, Skills, Abilities:
Have excellent oral and written communication skills, the ability to listen effectively, work independently, meet deadlines, and maintain confidentiality.
- Must be proficient in Word and Excel, and have the ability to organize and prioritize work.
