When you meet with our Client Care Coordinator - here's what you may experience:
Needs Assessment
The Client Care Coordinator will ask many questions to discover where your loved one could benefit from a helping hand, along with what aspects of daily life are going just fine right now.
Review of ADLs
Next, the Client Care Coordinator will ask questions about common Activities of Daily Living: Bathing, dressing, cooking, transferring in and out of chair or bed – those types of activities. Are they still manageable, or do they require assistance?
Identify Opportunities for Support
Once the Client Care Coordinator has assessed the situation, they will explore care options the family may benefit from, based on any gaps or deficiencies noted.
Safe Environment Assessment
Our Client Care Coordinators will discuss best practices for creating a safe environment, such as removing tripping hazards and lack of grab bars in wet areas – or any other safety concerns with you and your family.
Social Assessment
Next, the Client Care Coordinator will identify any physical or emotional risks associated with isolation and loneliness.
Care Plan
The Client Care Coordinator will then discuss any health concerns, medication management and other issues that should be addressed in a care plan.
Care Consult Recommendations
We will review the results with you and make recommendations. Together, you can discuss the hours and types of care that would best support your family.
