We're looking for a Client Care Coordinator ($50K - $60K/year +bonus opportunities)

Location: Home Instead Summit & Medina Countiies | Schedule: Monday–Friday, 8:30 am - 4:30 pm | In-person role

The Job

You're the go-to person for clients and families — the one who builds trust fast, solves problems on the spot, and keeps every moving piece of care on track.

  • Own the relationship. Be the primary point of contact for clients and families from day one.
  • Think fast, solve faster. Handle client concerns with empathy and quick, practical judgment.
  • Consult and close. Run care consultations, assess needs, and build custom care plans.
  • Keep it moving. Coordinate with schedulers, Care Pros, and healthcare pros so nothing slips.
  • Follow through. Own quality assurance visits, documentation, and follow-up — no dropped balls.
  • Spot what's next. Flag when a care plan needs to evolve and advocate for it.

What You'll Need

  • Client services, healthcare, senior care, social work, or care coordination experience, at least 3 years
  • Do you have at least two (2) years of care management experience? I.E. writing, editing plans of care?
  • College degree preferred
  • Microsoft Office proficiency; CRM/client management software a plus
  • Valid driver's license, current car insurance and reliable transportation
  • Availability for some evenings/weekends
  • High integrity and the ability to keep things confidential
