Please note: This position requires an active RN license. Non-nurses will not be considered.

What You’ll Do

• Conduct new client admissions and case openings
• Develop and implement individualized care plans in collaboration with physicians
• Ensure all services comply with NYSDOH and LHCSA regulations
• Oversee and maintain accurate clinical documentation and quarterly reviews
• Provide supervision, coaching, and support to Home Health Aides (HHAs)
• Lead staff education and in-service training programs
• Monitor and uphold state and federal compliance standards

What We’re Looking For

• Active New York State RN License
• Minimum 2 years of home care experience
• Strong knowledge of LHCSA regulations and NYSDOH standards
• Ability to work independently and manage responsibilities efficiently
• Excellent communication and leadership skills
