What You'll Do
As a Client Care Coordinator, you will be the primary advocate for our clients and their families, ensuring they receive exceptional care and support from the very first phone call through their entire Home Instead journey.
Client Care & Relationship Building
- Serve as a trusted advocate for clients, ensuring they receive the highest standard of care and support.
- Serve as the primary point of contact for clients and their families, building lasting, trust-based relationships.
- Conduct in-depth Care Consultations and service inquiries with potential clients and families, assessing needs, creating customized care plans, and executing the consultative sales process.
- Maintain consistent follow-up with potential clients and families who have not yet initiated services, ensuring ongoing support and engagement.
Care Coordination & Quality Assurance
- Create and maintain individualized care plans that meet client needs and goals.
- Coordinate client and Care Professional introductions to ensure successful care matches.
- Conduct routine quality assurance visits and client satisfaction check-ins.
- Collaborate with scheduling and caregiving teams to ensure seamless service delivery.
- Monitor changing client needs and adjust care plans accordingly.
Problem Solving & Client Advocacy
- Address concerns and service issues with professionalism, empathy, and urgency.
- Advocate for clients and identify opportunities to enhance services.
- Support positive outcomes for clients, families, and Care Professionals.
Documentation & Compliance
- Maintain accurate client records, assessments, care plans, and visit notes.
- Ensure compliance with company policies and industry standards.
- Complete all required documentation in a timely and accurate manner.
What We're Looking For
Experience
- Experience in client services, healthcare, home care, senior care, social work, case management, or a related field preferred.
- Experience working directly with older adults, families, or caregivers is highly valued.
- Strong customer service and relationship-building skills.
Education
- Bachelor's degree in Healthcare Administration, Social Work, Gerontology, Human Services, Nursing, or a related field preferred, but not required.
Technical Skills
- Proficiency with Microsoft Office (Word, Excel, Outlook).
- Experience with CRM, scheduling, or client management software preferred.
