RN - Client Care Team

Job Details

Objective:

The Client Care Nurse (RN) is expected to perform a variety of client services duties including revenue objectives. The Client Care Nurse trains and supervises caregivers in the provision of personal care services in accordance with State requirements. This employee will also conduct service callspersonal care assessments, and supervisory/quality assurance visits in order to provide the highest quality service to clients. In addition, the Client Care Nurse will generate revenue from a variety of duties, including researching and pursuing new referral sources as well as maintaining existing referral resources. This position is responsible for reporting results in moving relationships forward to Manager on a weekly basis, discussing successes and opportunities, and demonstrating documented progress and inquiry production in both client care and revenue generating areas.

Client Care Nurse Responsibilities:

  • Develop and maintain knowledge of brand. Effectively presents Home Care marketing materials and services to referral providers, including 1:1 meetings, as well as planning, arranging and conducting formal group presentations to referral providers.
  • Achieve inquiry generation objectives for assigned territory with the primary goal of bringing in new business to increase overall market share. Target accounts include, but are not limited to, hospitals, physicians, home health agencies, assisted living facilities, nursing homes, senior centers, hospice, funeral homes, etc.
  • Execute marketing plans, detailing weekly and monthly activities focused on meeting or exceeding established goals.
  • Research and develop client and referral relationships; performs all phases of sales process, including preparation and documentation of cold calls, follow-up calls, diagnosis of referral source and client needs, recommendation of appropriate Home Instead® services and additional community resources, maintaining and nurturing existing referral accounts, including follow-up, as necessary.
  • Conduct service calls and initial RN assessments with prospective/new clients and supervisory/QA visits with clients in person or over the phone according to the QA frequency guidelines.
  • Monitor, mediate, and log client activity and follow up on the assignments and client services.
  • Maintain accurate records of all client, referrals, prospecting and sales activities (i.e. calls, email, Web, presentations, face-to-face, follow-up activities) within the assigned territory in a database that can demonstrate ability to maximize territory potential.
  • Train in the provision of personal care services in accordance with State requirements as well as participate in and contribute to the development of educational programs offered to clients, prospects, and co-workers.
  • Counsel caregivers on supervisory/quality assurance concerns after visiting clients and provide RN supervision to address concerns as appropriate.

SPECIALIZED TRAINING AVAILABLE ALSO: Alzheimer, Hospice, Dementia, CPR, First Aide

Duties: include, but are not limited to:

  • Companionship and conversation
  • Light housekeeping tasks and meal preparation
  • Transportation to doctor appointments and errands (PAY: $0.50/mile when you use your car)
  • Medication and appointment reminders
  • Personal Grooming (i.e. dressing, shaving, dental care, bathing, toileting)
  • Dementia/Alzheimer’s Care

Requirements to be a CAREGiver:

  • Ability to treat and care for seniors and their property with dignity and respect
  • Ability to communicate with clients in a friendly and congenial manner
  • Must be at least 18 years old
  • Complete a criminal background check, DMV check and drug screen
  • Possess a valid driver’s license and valid auto insurance
  • For Personal Care (higher level of care/pay) assignments: current CNA license OR attend our in-house training
