Job Details

Home Instead Senior Care Vancouver is looking for CAREGivers in Vancouver with big hearts to become a part of our team of CAREgivers.  By providing a variety of non-medical services that allow seniors to remain in their home and meet the challenges of aging with dignity, care and compassion, we improve the quality of life of many seniors in our Vancouver community!   

We are currently in need of CAREGivers who have a valid BC driver's license, car and insurance - a mileage allowance is provided for transportation of our clients during shifts to cover related costs.   

Part Time to Full Time shifts available

Must be able to work Days, Evenings, Overnights and Weekends (or a mixture of both), ability to work on Sundays preferred

KEY DUTIES:

  • Provide Companionship, Home Helper and Personal Care services which may include***
    • Engaging with clients in conversation and activities 
    • Housekeeping tasks, laundry
    • Meal preparation, clean up
    • Medication and appointment reminders
    • Accompanying clients to activities and appointments
    • Assistance with walking and stabilization

The shifts are usually 12 hours - from 8pm to 8am. This shift is considered a sleeping shift, so you will be asleep for at least 6 hours of the shift. You will be required to help with getting the client to bed as well as morning routine. The shift may include helping with toileting during the night.

We are expanding our scope of services and looking for a confident practicing Registered Nurse to join our Vancouver office as our lead Client Care Manager!

Objective:

  • Ensure the ongoing satisfaction, quality, and safety of existing clients through regular contact, in conjunction with designated quality assurance home visits for clients who specifically use medication administration and nursing delegated specialized services.
  • The position requires the exercise of discretion and independent judgment regarding significant matters of quality assurances/clinical management/risk management and training of the CAREGiversSM for Home Instead Senior Care.
  • Clear understanding of RN regulations and responsibilities for British Columbia as related to nursing delegation to an unlicensed caregiver.

Primary Responsibilities:

  • Initiate and manage medication administration and/or nursing delegated specialized services to clients.
  • Maintain a current plan of care outlining the specific specialized services to be provided to the client.
  • Ensure proper maintenance of client journals: collect service records for office file and keep journal stocked with appropriate documentation forms.
  • Provide oversight, up to and including the instruction of initial and ongoing in-office and field training for CAREGivers.
  • Supervise ongoing case management of all patients ensuring an accurate evaluation and treatment plan, maintaining our quality model, and utilizing services appropriately to achieve optimal outcomes.
  • Communicate with clients and families as needed regarding satisfaction and/or issues of clients.
  • Maintains ongoing communication regarding patient care with physicians, referral sources, caregivers, patients, and associates to facilitate coordination of care. Assure physician approval of plans for continuity of care. Ability to communicate with patients of various levels of functioning and various types of personalities.
  • Develops relationships with referral partners, promoting the company and supporting sales and marketing initiatives
  • Develop short and long-term goals for the entire care department.
  • Establish new policies and update existing policies to improve the standard of care for clients.
  • Respond to any nursing-related issues in a timely manner.
  • Maintains effective and efficient ongoing communication with the senior management related to patient care, physician relations, community relations, or other areas impacting agency quality performance.
  • Provides performance feedback and coaching to direct report team members.
  • Adhere to established confidentiality standards and privacy guidelines concerning community and patient information.
  • Consistently demonstrates sound judgment in the day-to-day interactions with co-workers, management staff, and clients.
  • Maintain positive relationships and rapport with co-workers and community associates.
  • Supervise and participate in emergency on-call support.
  • Conduct ongoing in-office and field trainings for CAREGivers.
  • Conduct specialized services trainings with individual CAREGivers who are serving high needs clients receiving specialized services. The training must include a return demonstration performed by the CAREGiver and signed off/dated by the nurse. This documentation must be retained in the CAREGiver’s employment file. 
  • Act as an expert adviser regarding the medication services and delegated specialized services training given to CAREGiver.
  • Keep detailed documentation of all relevant phone and in-person contact with clients, client representatives, CAREGivers and others.

Senior Care Service Coordinator

Home Instead Senior Care Vancouver is seeking a Service Coordinator!

As a Home Instead Service Coordinator, you will be responsible for scheduling clients and CAREGiversSM and related support activities, with the goal of promoting the highest quality service to our clients and extraordinary relationships

Primary Responsibilities:

  • Create and maintain client and CAREGiver schedules with an emphasis on high quality matches, and effective utilization of CAREGiver personnel
  • Monitor, mediate, and log all client and CAREGiver activity utilizing the software system
  • Follow up with all client and CAREGiver issues to ensure their problems are resolved
  • Enter and maintain accurate client and CAREGiver records in the software system
  • Perform other functions as necessary to support our office and Home Instead clients and staff
    • Answer incoming calls from clients and service inquiries in a friendly, professional and knowledgeable manner
    • Demonstrate open and effective communication with the franchise owner, colleagues, CAREGivers, clients and family members
    • Participate as needed in all CAREGiver meetings

Our ideal candidate:

  • Demonstrates effective interpersonal, oral and written communication; must be fluent in English
  • Has excellent customer service, phone and listening skills
  • Has excellent organizational and decision making skills; is able to work independently, set priorities and meet deadlines
