5. Car Registration (if applicable)

7. Home Care Aid Registration (Fingerprint Background Check) ($125 Initial Cost $100 Reimbursement when training is complete)

She has partial incontinence and frequent accidents if she is not reminded to use the restroom. MONITOR eating habits, choke risk, and need to be encouraged to eat in small bites
PAY RATE FOR THIS CLIENT: $18-$20/HR
