Job Summary: The Registered Nurse (RN) at Home Instead plays a crucial role in providing comprehensive, compassionate, and professional care to our clients in their homes. As an RN, you will be responsible for assessing client needs, developing care plans, coordinating services, and ensuring the highest quality of care for our clients. You will work closely with the agency team, caregivers, and clients' families to promote the well-being and independence of our clients.

Duties and Responsibilities:

  1. Client Assessment: Conduct thorough assessments of clients' health status, medical history, and care needs to develop personalized care plans that align with their unique requirements and preferences.

  2. Care Planning: Collaborate with the client, their family, and caregivers to create individualized care plans that address health goals, safety concerns, and activities of daily living.

  3. Care Coordination: Coordinate and oversee all aspects of the client's care, ensuring seamless communication between caregivers, families, and other healthcare professionals involved in the client's care.

  4. Medication Management: Review and manage medication regimens, ensuring compliance and safety in accordance with physicians' orders and industry regulations.

  5. Health Monitoring: Monitor clients' health status regularly, identifying any changes or potential issues, and promptly communicating with appropriate healthcare professionals if necessary.

  6. Education and Support: Provide education and support to clients and their families on managing chronic conditions, health promotion, and maintaining a safe home environment.

  7. Training and Supervision: Train and supervise non-medical caregivers to ensure they are competent and confident in providing care in line with established protocols and care plans.

  8. Documentation: Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and any other required documentation in compliance with agency policies and state regulations.

  9. Compliance: Stay current with New Jersey state regulations and industry best practices to ensure the agency's operations and care delivery meet all applicable standards.

  10. Quality Assurance: Participate in quality assurance initiatives, ensuring the highest level of care and service delivery to clients.

Qualifications:

  • Current, valid RN license in the state of New Jersey.
  • Minimum of 2 years of nursing experience, preferably in home care, community health, or geriatrics.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities to work effectively with clients, families, and team members.
  • Knowledge of home care regulations and best practices.
  • Ability to adapt to different environments and handle various client needs with compassion and professionalism.
