Client Care Coordinator - MSW

Our Home Instead franchise is an Oregon licensed in-home care agency providing caregiving services to seniors in Beaverton, Tigard, Tualatin, and Sherwood. We are seeking an MSW to help manage our client caseload. This is a full-time position with significant time in the field meeting with clients and assessing their care needs.

We are a growing company and are looking to hire as soon as possible.

EDUCATION / EXPERIENCE REQUIREMENTS:

· Masters in Social Work Required

· One year experience required in home care, health care or senior-related industry. An equivalent combination of education and work experience may be considered.

· Must possess a valid driver’s license.

COMPENSATION / BENEFITS:

· Mostly Monday – Friday daytime work schedule for your work/life balance.

· We will reimburse you for 2 licensure supervision hours per month.

· We will reimburse you for your licensure exam fees.

· Mileage reimbursement or company vehicle provided for all field work.

· Annual salary of $43,000 to $49,000 depending on experience

OBJECTIVES:

· Meet with prospective clients to discover their needs, sign them up for service, and establish a billing relationship.

· Ensure satisfaction, safety, and quality care through regular phone contact with clients and representatives as well as live visits in their homes.

· Uncover opportunities to maintain and expand service hours while improving the overall value we provide to clients.

PRIMARY DUTIES:

· Be a primary contact in the office for clients and family members to facilitate communication and issue resolution.

· Conduct pre-service care consultations in the field to assess clients and start them on service.  

· Keep system documentation of all relevant phone and in-person contacts with clients, client representatives, CAREGivers and others.

· Ensure all quality assurance home visits are completed with new and ongoing clients as required by Oregon rule. Ensure proper maintenance of client journals. Collect service records for office file and keep journal stocked with blank forms.

· Maintain all active prospects and client records in Salesforce and ClearCare. Update and inactivate prospect and client files as needed.

· Take phone inquiries from prospective clients, set up care consultations, record client inquiries in Salesforce to enable reporting on client acquisition metrics.

· Know about long term care insurance to be in a position to advise clients on this topic during care consultations.

· Responsible for maintaining client files in both digital and hard copy formats.

· 90- Day audit of client files when Home Visit is conducted.

·Manage the service relationships with County and Federal government contracts (VA, Medicaid, FCSP, OPI) including receiving referrals, documenting our responses to referrals, setting up care consultations, and following up on contract details.

· Gain mastery of all the systems and software programs used by our office to maximize efficiency, effectiveness, and compliance.

SECONDARY DUTIES:

· Conduct CAREGiver to client introductions in client homes.

· Deep knowledge of all care consultation paperwork: Client Disclosure Statement, Client Assessment, Service Plan, Service Agreement, Medication Services Agreement, and Direct Payment Authorization.

· Assist occasionally with on-call or weekend duties as needed.

· Other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES:

· Must have an understanding of and uphold the policies and procedures established by MBCG, LLC, (d.b.a. an independently owned and operated Home Instead Senior Care franchise)

· Must demonstrate excellent oral and written communication skills and the ability to listen effectively.

· Must have the ability to work independently, maintain confidentiality of information and meet deadlines.

· Must demonstrate effective interpersonal skills as well as sound judgment and good decision-making.

· Must demonstrate discretion, integrity and fair-mindedness consistent with office standards, practices, policies and procedures.

· Must demonstrate knowledge of the senior care industry.

· Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work.

· Must have the ability to establish good working relationships with the general manager, office colleagues, CAREGivers, and the community

· Must have the ability to present a professional appearance and demeanor.

· Must have the ability to operate office equipment.

· Must be patient and congenial on the telephone.

· Must have computer skills and be proficient in Excel and Word.

· Must have the availability to work evenings or weekends as required.

· Must have the ability to work as a part of a team.
