Our Client Care Advisor is key to our company's growth. In this role, Key Responsibilities will be:

• Client Relationship Building & Engagement
  • Conduct care consultations with prospective clients
  • Develop personalized, individual plans of care for each client
  • Be the primary point of contact for clients and families
  • Follow up with potential clients to support their decision-making process
• Care Coordination & Quality Assurance
  • Collaborate with Care Pros and office staff to ensure seamless service
  • Lead client/Care Pro introductions and conduct regular quality assurance visits
  • Lead the Quality Assurance initiative for each client, ensuring successful care goal outcomes
  • Identify and recommend additional services based on evolving client needs
• Collaboration & Problem-Solving
  • Address concerns with empathy and professionalism
  • Work with families, Care Pros, and healthcare professionals to ensure optimal outcomes
• Compliance & Documentation
  • Maintain accurate client records and ensure compliance with policies and procedures

Requirements:

• 2 years demonstrable case management experience or LPN or RN, required
• Ability to work independently and make critical decisions without direct oversight
• Extraordinary time management, organizational, and prioritization skills
• Maintain regular attendance at the office to execute job responsibilities
• Strong Office 365 proficiency
• Valid Driver's License, automobile insurance
• Must successfully execute a pre-employment drug screen, criminal background check, and MVR check
