Aging Well Program
A wellness program built into our services that allows Home Instead to provide our clients with a proactive approach to aging well and staying in their home. Aging Well focuses
on quality of life and healthy living.
Setting Goals:
Clients and clinical nurse case managers collaboratively set goals across 8 points of wellness and develop strategies to strive toward by the next Quality Assurance visit within six months.
Nurse Advocacy & Resources:
Helping clients find and connect to home health community support & resources, as well as providing vetted referrals to help mitigate challenges.
Monitoring Progress:
Our clinical nurse case managers use a unique assessment & score tracking system at each Quality Assurance visit to monitor progress of every individual wellness area over time.

Our nurses and care professionals are always advocating, checking in, health coaching, and aligning wellness goals to keep clients safe and independent at home. Through our proactive clinical care management, every client receives:
- Clinical Advocacy – Nurses stay engaged in each client’s health plan and can communicate with doctors, rehabs, etc.
- Health Coaching – Encouraging healthy habits for long term success at home is a part of every care plan
- Aging Well Program – Setting goals across 8 points of wellness is aimed to help better understand and proactively care for a client's ever-changing needs.
