Our caregivers are trained to care for clients with specific chronic conditions such as diabetes, hypertension, arthritis, and depression, as well as dementia.

Level 3: Transitional Concierge Services
(Non-Medical Home Health)

Specially trained Care Providers work one-on-one to assist clients with:

  • Hospital-to-Home Discharge Coordination
  • Post-Surgical and/or Post-Hospitalization Care Management
  • Crisis Care
  • Hospice Care
  • Medication Supervision/Set-up
  • Ongoing Concierge Care Management**

(Includes all levels of care)

Scheduling Options/Additional Services

  • Minimum schedule – 3 hours 2x a week

HOSPITAL TO HOME DISCHARGE COORDINATION:

  • Development of Personalized Care Plan

SENIOR LIVING TRANSITION ASSISTANCE: progressive assistance with a gradual and smooth transition from independent living to an assisted living environment:

Financial Coverage for In-Home Care

Long-Term Care Insurance

Some long-term care insurance policies cover certain in-home care services. Check your policy to see if it covers the services you need at home. We work with many long-term care (LTC) insurance companies to bring you the covered services you need over the short- or long-term. We even compile and file the paperwork for you on a monthly basis.

VA Benefits

The Veterans Administration also offers financial programs that help pay for home care. Anyone with military service during a time of war is eligible for Aid & Attendance – criteria met by seventy five percent of all veterans, nearly 500,000 in Washington.
 
Veterans and their surviving spouses who require the assistance of another person –whether dressing, eating, taking medication, or who have a mental or physical incapacity – are eligible for additional monetary benefits, up to $2169 each month for married vets, up to $1830 for single vets and up to $1176 for surviving spouses.  In addition, Aid &Attendance covers care in Assisted Living and other long term care communities, as well as providing for home care at the veteran’s own home.

The Aid & Attendance application process itself is challenging and can be burdened by red tape. However, Veterans Service Officers are available through local VA offices to help you through the process.  In the Antelope Valley, call the Department of Military Affairs in Lancaster at 661-974-8842.  And in the Santa Clarita Valley, call the Santa Clarita Veteran Center, 661-670-8680.
