Transportation Services for Seniors

How Do We Do This?

Home Instead Senior Care and Care Connection work closely together to provide:

1. Appropriate, safe and timely transportation from the facility to the client's home.
2. Assistance getting the client comfortably settled at home, including;
3. Picking up medications from the local pharmacy
4. Full in-home care consultation and needs assessment with professional recommendation for continued care when appropriate.
5. Home Safety Assessment if needed.
6. Immediate Personal care needs, i.e., bathing, dressing, grooming, mobility, eating, etc.
7. Scheduled transportation to follow up medical appointments.
