​Aging Well Prescott provides care and support to older adults and their families.  Aging Well Prescott is focused on helping senior loved ones live as independently as possible while being safe and secure.   Aging Well Prescott was founded by Janice Mauz, RN, BSN, CMC Aging Well Prescott LLC (formerly JHM Senior Advocacy LLC).  Janice earned a Bachelor of Science (BSN) in Nursing from Westminster College in Salt Lake City, Utah.  She has a Certificate in Gerontology from Yavapai College in Prescott as well certification as a professional care manager (CMC).   Janice has lived and worked as an RN and care manager in the Quad Cities for over 25 years.

Services offered include:

  • Initial Assessment -- To identify area of concerns.  The assessment will include a review of health care status, physical functioning as well as cognitive status.
  • Education and advocacy – Aging Well Prescott offers in-home family consultations to provide detailed information, strategies and specific referrals to address known problems. 
  • Care management and support -- Aging Well Prescott offers support and advocacy and coordination of care for older adults and their families.

CPCC does not recommend any particular choices.   Rather, we encourage individuals to think about what they would like for their end-of-life choices and then discuss them with their loved ones, as well as to complete the appropriate documents necessary for their situation.  We encourage the completion of an Advance Directive form and a discussion of the documented wishes for end-of-life with family and loved ones.   If you need information on Advance Directives or would like more information about CPCC please contact us at 928-493-2394.

Prescott Meals on Wheels serves residents of the Prescott community who may find themselves challenged to live independently in the short or long term.  We assist adults 18 years of age or older who may have a temporary or permanent disability and struggle to live on their own. There are two primary criteria for program eligibility.  First, is the individual able to get to the store to purchase food or ask family or friends to bring groceries to them? Secondly, does the individual have a difficult time preparing food because of a physical disability and / or a cognitive limitation?  If the answer to one or both of these questions is yes, Prescott Meals On Wheels may be able to help with a weekday delivery of a hot, nutritious meal.
