Hospice Care Services at Home 

Hospice care is team-oriented care for people with a life-limiting illness or injury, most often called for when there is a life expectancy of six months or less. Hospice care is typically a team approach that includes medical care, pain management, emotional and spiritual support, and support to a dying person's loved ones. A hospice team can include doctors, nurses, home health aides, hospice workers, grief counselors, chaplains and family members.
