Home Instead's personal care aid-trained CAREGivers offer meal preparation and nutrition monitoring, light housekeeping and laundry, transportation for errands and doctor's appointments, medication reminders, assistance with bathing and dressing, safety supervision and activity/exercise encouragement, reducing the risk of hospitalization to 12.5%**.
The partnership between C3 Nexus and Home Instead Senior Care offers clients and their families high-tech and high-touch in-home care through six specific aspects of daily living: nurse concierge monitoring, BodyGuardian remote monitoring, cardiometabolic blood analysis, lifestat management system, care coordination portal and in-home companion and personal care.
C3 Nexus's clinically trainted RNs monitor clients and cooridinate targeted daily management to avert hospitalizations through face-to-face visits, social assessments, medication reconciliation and monitoring, nutrition reconciliation programs and customized fitness advice.
The small, non-invasive and FDA-cleared BodyGuardian device monitors ECG, heart rate, respiration rate, and 3D accelerometry to measure posture, position, movement and activity of the client. All such vitals are transmitted in real time to C3 Nexus' control center.
