What will you do as a Caregiver?

  • Provide companionship and conversation.
  • Shopping, errands, and appointments.
  • Light housekeeping and meal prep.

What do you need to be a Caregiver?

  • Caregiving experience (personal or professional), including hands-on personal care
  • Be 18 years or older
  • Have a valid Oregon driver’s license and a reliable vehicle
  • Pass a background check, DMV check and drug screen (including Marijuana)

Home Instead Senior Care
is looking for a caring and compassionate person to become a part of our team!


Our Mission:
To enhance the lives of aging adults throughout our community
and provide a variety of non-medical services that allow seniors to remain in their home
and meet the challenges of aging with dignity, care and compassion.

 
Home Instead Senior Care is an in-home care franchise that provides care to seniors in Marion and Polk counties. We are looking for a second part time RN to oversee the medication management of our Clients. 

The RN is expected to perform a variety of duties including teaching, initiating and overseeing medication services. The RN will delegate to our CAREGivers and perform 90-day in-home medication reviews. They are responsible to know the OAR's related to in-home care, OSBN regulations, and Home Instead Senior Care Standards.

Qualifications:
  • Current valid State of Oregon license to practice as an RN
  • Minimum 2 years’ experience as a nurse
  • Must possess a valid Oregon Drivers’ License
  • Retired RN’s encouraged to apply

Knowledge, Skills and Abilities:
  • Responsible for following all State and Federal regulations and company policies and procedures
  • Needs to be self-directed, a teacher, problem solver, great communicator and able to think and work independently
  • Must have excellent written and oral communication skills and the ability to listen effectively
  • Must have ability to establish good working relationships with administrative Staff, Clients, and CAREGivers
  • RN must achieve competency to delegate and supervise per OSBN

Essential Functions for Medication Services
RESPONSIBLE FOR:
  • Developing and implementing safe and appropriate medication delivery systems
  • Teach mandatory 4.5 hour initial medication training class to all new hire CAREGivers assuring that it meets all Home Instead and OAR regulations
  • Assure 1 hour annual medication training by reviewing and approving online classes or doing in-person class
  • Monitor medication administration by CAREGivers to determine further or additional changes to medication teaching
  • Ensure provision of written instructions for each type of medication by developing policy and procedures for all medication administration types
  • Develop and update all medication forms as needed, assuring they meet Home Instead and OAR compliances
  • Conduct assessments of Clients requesting medication services, in the home or by review of Client information, and information from Staff with experience with Client
  • Enter medication services in software program and update needed Plan of Care forms in paper file for State of Oregon
  • Consult with primary care providers, pharmacists, healthcare professionals and hospice providers on medication management as needed
  • Assure that all new, changed, or dc’d medication orders are documented and confirmed by agency Staff and transmitted within 72 hours to doctor for signature
  • Assure signed medication orders for all medications are received, reviewed and filed in Client office file within 30 days
  • Communicate medication services and medication changes to Admin Staff and assigned CAREGivers
  • Review Client medication administration records monthly for accuracy and completeness and file in chart after any indicated corrections completed by CAREGivers
  • Mentor, coach, and counsel CAREGivers and Admin Staff concerning medication services
  • The RN will perform quarterly medication visits to Client’s homes
  • Review all medication error reports 
  • Do home visits for medication concerns as needed

Essential Functions for RN Delegation:
  • Delegate nursing tasks to unlicensed persons according to the Oregon State of Nursing Nurse Practice Acts
  • Teach delegated nurse procedure to unlicensed Staff and assure comprehension by unlicensed Staff
  • Ensure written instructions regarding delegation of nursing procedure to unlicensed persons
  • Ensure all RN delegations forms completed and filed in Client chart

Other:
  • Be able to share 24/7 on-call
  • Be available to attend Quarterly Safety Committee meetings to review Incident Reports or medication errors
  • This will be a flexible schedule beginning about 12-15 hours/week and growing up to approximately 18-20 hours/week
