VA Referral Information for Home Based Primary Care services in Northwest Arkansas:

1.     Veteran must be enrolled in the VA healthcare system at VA Medical Center in Fayetteville and have an assigned Primary Care Physician. 

2.     Veteran must request aid from the VA Primary Care Physician for homemaker, respite, home health or personal care services.  The physician will put in a referral to the VA social work department who will assess need and determine level of care. 

3.     Veteran should request Home Instead Senior Care as the service provider.
