Quality Assurance
We are committed to providing responsive, quality care for each of our clients. Our goal is not only to meet your expectations, but to exceed them.
Vital pieces of our unique "Quality Assurance Program" include:
-
Our Client Care Specialist completes an initial home visit and creates an individualized "Plan of Care", based on the client's needs, likes and dislikes.
-
Our Client Care Specialist meets with a Staff Coordinator to communication the "Plan of Care" and important compatibility criteria.
-
Our Staff Coordinator assigns qualified CAREGiver(s), manages the overall care schedule and maintains communications regarding the schedule with the client and their family.
-
Our Client Care Specialist conducts initial CAREGiver introduction with client specific training and follows up with client and/or family within first few days of service.
-
The CAREGiver records their daily activities and observations in the Client Journal.
-
Our Client Care Specialist and R.N./Nurse Consultant conduct regular "home visits" to see first hand how the client's needs are being met, if there are changes in the client's condition and needs, or if there are other additional services or equipment that may now be appropriate.
-
In Addition, the Staff Coordinator conducts regular phone calls to assure that our CAREGivers are providing quality care.
-
Our Professional CAREGiver notifies the office if they notice any significant changes in the client's physical, mental or emotional health, so we may communicate this to the client's family or other loved ones. When these situations arise, our Client Care Specialist conducts a special "home visit" to assess the situation.
